Indications

  • Treatment of hypertension
  • Renal protection in type-2 diabetic patients with proteinuria

Pharmacology

  • Non-peptide orally active angiotensin II receptor blocker
  • Binds to AT1 receptor
  • Reduces vasoconstriction and aldosterone release

Dosage & Administration

  • Starting and maintenance dose of 50 mg once daily
  • Dosage range from 25 mg to 100 mg
  • Different dosages for patients above 75 years

Interaction

  • Reduced levels with rifampicin and fluconazole
  • Potentiation with hydrochlorothiazide
  • Increased serum potassium with potassium-sparing diuretics
  • Attenuated effect with indomethacin
  • Increased risk of renal impairment with certain drug combinations

Contraindications

  • Contraindicated in pregnant women
  • Contraindicated in patients hypersensitive to any component
  • Should not be administered with Aliskiren in patients with diabetes

Pregnancy & Lactation

Precautions & Warnings

  • Reduces fetal renal function in pregnancy
  • Symptomatic hypotension in intravascularly volume-depleted patients
  • Increased plasma concentration in cirrhotic patients
  • Changes in renal function reported in renal-impaired patients

Use in Special Populations

  • No initial dosage adjustment in patients with mild renal impairment
  • Lower starting dose for patients with moderate to severe renal impairment or on dialysis
  • Lower dose for patients with a history of hepatic impairment
