Indications

  • First-line therapy for metastatic carcinoma of the colon or rectum
  • Treatment for recurrent or progressing metastatic carcinoma of the colon or rectum

Pharmacology

  • Derivative of Camptothecin
  • Interacts with topoisomerase I
  • Induces reversible single-strand breaks in DNA
  • Active metabolite SN-38 binds to the topoisomerase I-DNA complex

Dosage & Administration

  • Colorectal cancer combination regimen 1: Irinotecan 125 mg/m2 intravenous infusion over 90 minutes on specific days
  • 5-FU and Leucovorin in specified combination regimens
  • Colorectal cancer single agent regimen 1: Irinotecan 125 mg/m2 intravenous infusion over 90 minutes on specific days followed by 2-week rest

Interaction

  • Diuretics increase risk of dehydration
  • Prophylactic dexamethasone as an antiemetic may enhance lymphocytopenia
  • St John's wort and ketoconazole may reduce irinotecan exposure

Precautions & Warnings

Use in Special Populations

  • Effectiveness not established in pediatric patients
  • Greater risk of early and late diarrhea in patients over 65 years
  • Use with caution in patients with renal impairment
  • Diminished clearance in hepatic impairment
