Indications
- First-line therapy for metastatic carcinoma of the colon or rectum
- Treatment for recurrent or progressing metastatic carcinoma of the colon or rectum
Pharmacology
- Derivative of Camptothecin
- Interacts with topoisomerase I
- Induces reversible single-strand breaks in DNA
- Active metabolite SN-38 binds to the topoisomerase I-DNA complex
Dosage & Administration
- Colorectal cancer combination regimen 1: Irinotecan 125 mg/m2 intravenous infusion over 90 minutes on specific days
- 5-FU and Leucovorin in specified combination regimens
- Colorectal cancer single agent regimen 1: Irinotecan 125 mg/m2 intravenous infusion over 90 minutes on specific days followed by 2-week rest
Interaction
- Diuretics increase risk of dehydration
- Prophylactic dexamethasone as an antiemetic may enhance lymphocytopenia
- St John's wort and ketoconazole may reduce irinotecan exposure

Precautions & Warnings
- Early diarrhea and cholinergic reactions

- Increased risk for neutropenia in individuals with reduced UGT1A1 activity
Use in Special Populations
- Effectiveness not established in pediatric patients
- Greater risk of early and late diarrhea in patients over 65 years
- Use with caution in patients with renal impairment
- Diminished clearance in hepatic impairment
