Indications
- Neutralising the immediate toxic effects of folic acid antagonists
- Calcium folinate rescue treatment for minimising systemic toxicity
- Treatment of megaloblastic anaemias due to various causes
Composition
- 6.249 mg Calcium Folinate Hydrate BP equivalent to 5 mg Folinic Acid per tablet
- 18.746 mg Calcium Folinate Hydrate BP equivalent to 15 mg Folinic Acid per tablet
Pharmacology
- Calcium Folinate Hydrate as a metabolite and active form of folic acid
- Cofactor for 1-carbon transfer reactions in nucleic acid biosynthesis
- Potent antidote for toxic effects of folic acid antagonists
Dosage & Administration
- Initiation of calcium folinate rescue within 24 hours of methotrexate administration
- Dose regimens based on the administered dose of methotrexate
- Dosage for countering hematologic toxicity from folic acid antagonists with less affinity for dihydrofolate reductase
Interaction
- Potential counteraction of antiepileptic effect with large amounts of folinates
- Caution with concurrent administration with fluoropyrimidine
Contraindications
- Known hypersensitivity to calcium folinate or any other components
- Pernicious anemia or other megaloblastic anemia with deficient vitamin B12
- Rare hereditary problems of galactose intolerance, lapp lactase deficiency or glucose-galactose malabsorption

Pregnancy & Lactation
- Limited clinical studies on use in pregnant women
- Caution advised for administration to nursing mothers
Precautions & Warnings
- Should only be used with methotrexate or 5-FU under supervision of experienced clinician
- Administered as soon as possible in the treatment of inadvertent overdosage of folic acid antagonist
- Avoid simultaneous administration with folic acid antagonists like methotrexate
Overdose Effects
- Potential nullifying of chemotherapeutic effect of folic acid antagonists
- No specific antidote to calcium folinate overdose
