Indications
- In combination with cisplatin for first-line treatment of patients with locally advanced or metastatic non-small cell lung cancer (NSCLC)
- As a single agent, for the treatment of patients with metastatic NSCLC
Pharmacology
- Binds to tubulin and inhibits microtubule formation
- Disrupts the formation of the mitotic spindle thereby arresting the cell at metaphase
Dosage & Administration
- Intravenous (Adult): Cervical cancer: 30 mg/m^2 days 1 and 8 of a 21-day treatment cycle
- Breast cancer, Ovarian cancer: 25 mg/m^2 dose every 7 days
- Non-small cell lung cancer: As single agent - 30 mg/m^2 weekly as infusion or as slow bolus over 5-10 minutes, As combination therapy with cisplatin: 25-30 mg/m^2 every 7 days
- Oral (Adult): Non-small cell lung cancer: 60 mg/m^2 once weekly for 3 weeks, may increase subsequently to 80 mg/m^2 once weekly
Interaction
- Increased risk of granulocytopenia with cisplatin
- Increased risk of neurotoxicity with paclitaxel, itraconazole, ketoconazole
- Increased radiosensitizing effects with prior or concomitant radiation therapy
Contraindications
- Hypersensitivity to vinorelbine or other vinca alkaloids
- Severe current or recent infection
- Neutropenia
- Thrombocytopenia
- Severe hepatic impairment
- Intrathecal admin
- Do not give concomitantly with radiotherapy if liver is in treatment field
- Pregnancy, lactation
Side Effects
- Neurotoxicity
- Peripheral paraesthesia
- Loss of deep tendon reflexes
- Abdominal pain
- Severe constipation
- Diarrhea
- Alopecia
- Dose limiting granulocytopenia

Precautions & Warnings
- Hepatic impairment
- Compromised bone marrow reserve
- Prior radiation therapy
- CBC with differentials to be monitored prior to admin of subsequent doses
- Each admin to be followed by at least 250 ml of normal saline to flush the vein
- Avoid extravasation
Use in Special Populations
- Dose adjustments may be needed for patients with massive liver metastases
