Indications
- Treatment of advanced carcinoma of the ovary
- Adjuvant treatment of node-positive breast cancer

- Treatment of non-small cell lung cancer
- Second-line treatment of AIDS related Kaposi's Sarcoma
- Treatment of Gastric Carcinoma
Pharmacology
- Promotes assembly of microtubules
- Stabilizes microtubules
- Inhibits normal dynamic reorganization of the microtubule network
- Induces abnormal arrays of microtubules
- Causes multipole asters of microtubules during mitosis
- Plasma concentrations decline in a biphasic manner
Dosage & Administration
- Premedication required before administration
- Different dosages and durations for various cancer treatments
- Recommended dose adjustments based on individual patient tolerance
Interaction
- Cimetidine does not affect Paxel clearance
- Cisplatin treatment affects Paxel clearance
- Sequence effects observed with Paxel and doxorubicin combination use
- Metabolism catalyzed in part by cytochrome P450 isoenzymes
- Caution required when administering Paxel concomitantly with certain medicines
Contraindications
- Severe hypersensitivity reactions to paclitaxel
- Contraindicated during lactation
- Not to be used in patients with specific baseline neutrophil and platelet counts
- Contraindicated in patients with severe hepatic impairment
- Contraindicated in AIDS patients with concurrent serious, uncontrolled infections
Side Effects
- Low blood counts
- Increased risk for infection, anemia, and/or bleeding
- Hair loss
- Arthralgias and myalgias
- Peripheral neuropathy
- Mild to moderate nausea and vomiting
- Diarrhea
- Mouth sores
- Hypersensitivity reactions
- Facial flushing
- Chills
- Shortness of breath
- Hives
- Edema
- Liver problems
- Low blood pressure
- Darkening of the skin

Precautions & Warnings
- Should be administered under the supervision of an experienced physician
- Significant hypersensitivity reactions may occur
- Close monitoring of infusion site for possible infiltration required
- Severe bone marrow suppression, primarily neutropenia, is the dose-limiting toxicity
- Severe cardiac conduction abnormalities reported rarely
Use in Special Populations
- Dose adjustment for patients with severe neutropenia or severe peripheral neuropathy
- Usage not recommended in children below 18 years due to lack of data on safety and efficacy
Overdose Effects
No known antidote for Paxel overdose
Reconstitution
Paxel Injection must be diluted prior to infusion
Storage Conditions
Store below 30°C, protect from light, and use only freshly prepared solution
