Indications
- Treatment of anemia in Chronic Kidney Disease (CKD) patients on dialysis and not on dialysis
- Treatment of anemia in HIV-infected patients on Zidovudine
- Reduction of allogeneic RBC transfusions in patients undergoing elective, noncardiac, nonvascular surgery
- Prevention of anemia of prematurity in infants with a birth weight of 750 to 1500 gm and gestational age of less than 34 weeks
Description
Recombinant human erythropoietin (EPO) expressed in Chinese hamster ovary cells with a 165 amino acid sequence identical to human urinary EPO
Dosage
- Evaluation of Iron Stores and Nutritional Factors before and during treatment
- Monitoring hemoglobin levels at least weekly until stable, then at least monthly
- Dose adjustments based on hemoglobin rate of rise, rate of decline, and ESA responsiveness
- Different starting doses and dose adjustments for CKD patients on dialysis and not on dialysis, Zidovudine-treated HIV-infected Patients, and Cancer Patients on Chemotherapy
Administration
- Do not shake or freeze
- Visual inspection for particulate matter and discoloration before administration
- Protection from light
- Discard unused portions
Interaction
Potentiation by therapeutic administration of a haematinic agent like ferrous sulphate when a deficiency state exists
Contraindications
- Uncontrolled hypertension
- Serious allergic reactions to Erythropoietin alfa
- Pure Red Cell Aplasia (PRCA) following treatment with any erythropoietin
Side Effects
- Hypertension, arthralgia, muscle spasm, pyrexia, dizziness, medical device malfunction, vascular occlusion, and upper respiratory tract infection in CKD patients
- Pyrexia, cough, rash, and injection site irritation in Zidovudine-treated HIV-infected Patients
- Nausea, vomiting, myalgia, arthralgia, stomatitis, cough, weight decrease, leukopenia, bone pain, rash, hyperglycemia, headache, depression, dysphagia, hypokalemia, and thrombosis in Cancer Patients on Chemotherapy
- Nausea, vomiting, pruritus, headache, injection site pain, chills, deep vein thrombosis, cough, and hypertension in Surgery Patients
- Fall in serum ferritin values in premature infants
Pregnancy & Lactation
- Caution in pregnancy due to potential risks
- Caution in nursing women, decision is based on benefit to the child and woman
Overdose Effects
Manage with discontinuation or reduction of dosage and/or with phlebotomy, as clinically indicated; Severe hypertension observed in overdose cases
