Indications

  • Improves glycemic control in adults with type 2 diabetes mellitus

Pharmacology

  • Inhibition of pancreatic alpha-amylase and intestinal alpha-glucosidase enzymes
  • Delayed glucose absorption
  • Lowering of postprandial hyperglycemia
  • Additive effect with sulfonylureas, insulin, or metformin
  • Diminished insulinotropic and weight-increasing effects of sulfonylureas
  • No inhibitory activity against lactase

Dosage & Administration

  • Recommended starting dosage of 25 mg orally three times daily
  • Gradual dose titration to minimize gastrointestinal side effects
  • Dosage adjustment based on glucose levels and tolerance
  • Maintenance dosage ranges from 50 mg t.i.d. to 100 mg t.i.d.

Interaction

  • Certain drugs may produce hyperglycemia and lead to loss of blood glucose control
  • Observation required when drugs are administered or withdrawn in combination with sulfonylureas or insulin

Contraindications

  • Hypersensitivity to the drug
  • Diabetic ketoacidosis or cirrhosis
  • Inflammatory bowel disease, colonic ulceration, intestinal obstruction, or predisposition to intestinal obstruction
  • Chronic intestinal diseases associated with digestion or absorption disorders
  • Conditions deteriorating due to increased gas formation in the intestine

Pregnancy & Lactation

  • Safety not established in pregnant women

Precautions & Warnings

  • Unlikelihood of causing hypoglycemia when administered alone
  • Increased potential for hypoglycemia when used in combination with sulfonylureas or insulin
  • Use of oral glucose instead of sucrose for hypoglycemia treatment
  • Severe hypoglycemia may require intravenous glucose infusion or glucagon injection

Use in Special Populations

  • Pediatric use safety and effectiveness not established
  • No overall differences in safety and effectiveness observed in geriatric use

Overdose Effects

  • No hypoglycemia resulting from overdose
  • Transient increases in flatulence, diarrhea, and abdominal discomfort
  • Avoidance of carbohydrates for 4-6 hours in case of overdosage
