Indications

  • Treatment of essential hypertension as monotherapy
  • Treatment of angina pectoris as monotherapy
  • Treatment of vasospastic angina
  • May be used in combination with other antihypertensive or antianginal agents

Pharmacology

  • Dihydropyridine calcium-channel blocker
  • Long duration of action
  • Influences myocardial cells and vascular smooth muscle
  • Results in vasodilation and reduced peripheral resistance
  • Increased coronary blood flow and myocardial oxygen supply

Dosage & Administration

  • Usual dose of 5mg once daily for hypertension
  • Maximum dose of 10mg once daily for hypertension
  • Lower starting dose for elderly or hepatically impaired patients
  • Dosage of 5-10mg for angina (chronic stable or vasospastic)

Interaction

  • Potential hazardous interactions with p-blockers in severely impaired cardiac patients
  • No interaction with Digoxin
  • No interaction with Cimetidine
  • No significant alteration of Warfarin effect
  • No influence of food on absorption

Contraindications

  • Hypersensitivity to dihydropyridine derivatives
  • Contraindicated in pregnant women

Side Effects

  • Common adverse effects include dizziness, flushing, headache, hypotension, peripheral edema
  • Gastrointestinal disturbances, increased micturition frequency, lethargy
  • Paradoxical increase in ischaemic chest pain
  • Rashes, fever, abnormalities in liver function

Pregnancy & Lactation

  • Pregnancy Category C
  • Use during pregnancy only if benefits outweigh risks
  • Unknown excretion in human milk
  • Discontinue nursing while on Amlodipine

Precautions & Warnings

  • Risk in patients with hepatic impairment, pregnancy, and breastfeeding

Overdose Effects

  • Symptoms of excessive peripheral vasodilatation and hypotension
  • Management with cardiovascular support, vasoconstrictors, calcium gluconate
  • Use of charcoal and gastric lavage in some cases
