Indications
- Treatment of hypertension alone or with other antihypertensive agents
- Lowering blood pressure
- Initial therapy for patients likely to need multiple antihypertensive agents
Pharmacology
- Dihydropyridine calcium channel blocker
- Inhibits transmembrane influx of calcium ions into vascular smooth muscle
- Acts directly on vascular smooth muscle to reduce peripheral vascular resistance
- Blocks vasoconstrictor and aldosterone-secreting effects of angiotensin II
- Reversible competitive inhibitor of the AT1 receptor
Dosage & Administration
- Substitute individually titrated components for patients on Amlodipine and Olmesartan Medoxomil
- Initiate with 5/20 mg once daily and titrate as needed up to a maximum of 10/40 mg once daily
- Recommended starting dose of Amlodipine is 2.5 mg for patients 75 years and older
Interaction
- Antihypertensive effect may be attenuated by NSAIDs including selective COX-2 inhibitors
- Close monitoring of blood pressure, renal function, and electrolytes required for patients on combination therapy
Contraindications
- Cannot be co-administered with Aliskiren in patients with diabetes

Pregnancy & Lactation
- Pregnancy Category D
- Not recommended for use in 2nd and 3rd trimester
- Discontinuation advised when pregnancy is detected
- Unknown excretion in human milk, decision should be made whether to discontinue nursing or discontinue the drug
Precautions & Warnings
- Caution in volume- or salt-depleted patients
- Risk of hypotension, vasodilation, and angina in specific patient populations
Use in Special Populations
- Safety and effectiveness not established in pediatric patients
- No overall differences in safety or effectiveness observed in subjects 65 years of age or older
- No studies in patients with renal impairment
- Initial therapy not recommended in hepatically impaired patients
