Indications

  • Treatment of hypertension
  • Lowering blood pressure
  • Initial therapy for patients likely to need multiple antihypertensive agents

Pharmacology

  • Dihydropyridine calcium channel blocker
  • Inhibits transmembrane influx of calcium ions
  • Acts on vascular smooth muscle
  • Acts on cardiac muscle
  • Peripheral arterial vasodilator
  • Blocks vasoconstrictor and aldosterone-secreting effects of angiotensin II
  • Selective inhibitor of the AT1 receptor

Dosage & Administration

  • Initial dose of 5/20 mg once daily
  • Titration up to a maximum of 10/40 mg once daily
  • Lower initial dose recommended for elderly patients

Interaction

  • Potential attenuation by NSAIDs including selective COX-2 inhibitors
  • Close monitoring of blood pressure, renal function, and electrolytes

Contraindications

  • Co-administration with Aliskiren in patients with diabetes

  • Peripheral edema
  • Headache
  • Flushing
  • Dizziness
  • Sprue-like enteropathy

Pregnancy & Lactation

  • Pregnancy Category D
  • Should not be used in 2nd and 3rd trimester
  • Potential excretion in human milk

  • Caution in volume- or salt-depleted patients
  • Caution in patients with severe aortic stenosis
  • Caution in patients with severe obstructive coronary artery disease

Use in Special Populations

  • Safety and effectiveness not established in pediatric patients
  • No overall differences in safety or effectiveness in patients 65 years or older
  • No studies in patients with renal impairment
  • Initial therapy not recommended in hepatically impaired patients
