Indications
- Treatment of hypertension
- Lowering blood pressure
- Initial therapy for multiple antihypertensive agents
Pharmacology
- Dihydropyridine calcium channel blocker
- Inhibits transmembrane influx of calcium ions
- Peripheral arterial vasodilator
- Selective blocker of AT1 receptor
Dosage & Administration
- Initial dosage of 5/20 mg once daily
- Titration to a maximum of 10/40 mg once daily
- Individual titration for patients on Amlodipine and Olmesartan Medoxomil
Interaction
- Potential attenuation by NSAIDs including selective COX-2 inhibitors
- Close monitoring of blood pressure, renal function, and electrolytes
Contraindications
- Co-administration with Aliskiren in patients with diabetes

- Dizziness
- Sprue-like enteropathy
Pregnancy & Lactation
- Pregnancy Category D
- Avoid use in 2nd and 3rd trimester
- Potential excretion in human milk
Precautions & Warnings
- Risk of hypotension in volume- or salt-depleted patients
- Risk of vasodilation in patients with severe aortic stenosis
- Risk of increased frequency, duration, or severity of angina or acute MI in patients with severe obstructive coronary artery disease
Use in Special Populations
- No established safety and effectiveness in pediatric patients
- No overall differences in safety or effectiveness observed in subjects 65 years or older
- No studies in patients with renal impairment
- Initial therapy not recommended in hepatically impaired patients
