Pharmacology

  • Bacitracin inhibits bacterial cell wall synthesis
  • Active against gram-positive bacteria
  • Active against some gram-negative species
  • Neomycin binds to bacterial ribosome
  • Inhibits protein synthesis
  • Active against gram-negative aerobes
  • Active against some strains of staphylococci

Dosage & Administration

  • Ointment: Apply 2-3 times daily
  • Ointment: Clean affected area before application
  • Ointment: Remove pus or crusts from affected area
  • Dusting Powder: Non-sterile dusting powder
  • Dusting Powder: Light dusting applied up to 4 times daily

Interactions

  • Bacitracin Zinc: Increased risk of nephrotoxicity with other nephrotoxic drugs
  • Bacitracin Zinc: May enhance the action of neuromuscular-blocking agents
  • Neomycin Sulphate: Additive nephrotoxic and neurotoxic effect with other aminoglycosides
  • Neomycin Sulphate: Enhanced toxicity with potent diuretics
  • Neomycin Sulphate: May impair absorption of other drugs

Contraindications

  • Hypersensitivity to any components

Side Effects

  • Well tolerated in most cases
  • Rare allergic reactions

Pregnancy & Lactation

  • Safe use not established during pregnancy & lactation
  • Use during pregnancy requires assessment of potential benefit vs. risk to fetus

Precautions & Warnings

  • Avoid use on large open wounds or severely injured skin
  • Avoid use on areas exuding large volumes of fluid
  • Avoid use on perforated eardrum due to risk of ototoxicity
  • Consult physician for deep puncture wounds, animal bites, or serious burns
  • Discontinue use and consult physician if condition persists or worsens
  • Avoid use for longer than 1 week without physician's direction

Use in Special Populations

  • Suitable for use in children at adult dose
  • Dose should be reduced in infants
  • Not recommended for neonates
