Description
- Baclofen is indicated in spasticity resulting from multiple sclerosis, flexor spasms and concomitant pain, clonus and muscular rigidity, skeletal muscle spasm resulting from rheumatic disorders, spinal cord injuries and other spinal cord diseases, cerebrovascular accidents or neoplastic or degenerative brain disease.
Dosage
- 5 mg 3 times daily for adults & children over 10 years
- 2.5 mg (2.5 ml) given 4 times daily for children under 10 years, then raised according to requirement
- Maximum dosage of 100 mg daily for adults & children over 10 years
- 10-20 mg (10-20 ml) for children aged 12 months to 2 years
- 20-30 mg (20-30 ml) for children aged 2 to 6 years
- 30-60 mg (30-60 ml) for children aged 6 to 10 years
Pharmacology
- Baclofen inhibits monosynaptic and polysynaptic reflexes at the spinal level by stimulating GABA receptors, which inhibits the release of glutamate and aspartate
- May also act at intraspinal sites producing CNS depression
- Exerts an antinociceptive effect
Interaction
- Increased sedation may occur if Baclodol is taken with agents acting on the central nervous system, alcohol or synthetic opiates
- Combined treatment with Baclodol and antihypertensives is likely to increase the fall in blood pressure
- Potentiation of pharmacological effects with concomitant administration of Baclodol and tricyclic antidepressants
- Reports of mental confusion, hallucinations, headaches, nausea and agitation in patients with Parkinson's disease receiving treatment with Baclodol and levodopa and carbidopa
- Concurrent use of MAO inhibitors and Baclodol may result in increased CNS depressant effects
- Caution should be exercised when administering Baclodol and magnesium sulphate or other neuromuscular blocking agents

Pregnancy & Lactation
- Pregnancy category B3
- Safe use during pregnancy has not been established
- Crosses the placental barrier
- Should only be administered to pregnant women when the potential benefits outweigh the possible hazards
- Excreted in breast milk, quantities are so small that no undesirable effects on the infant would be expected
Precautions & Warnings
- Lower doses should be used for patients with impaired renal function or those undergoing chronic haemodialysis
- Caution needed in patients with psychotic disorders, schizophrenia, depressive or manic disorders, or confusional states
- Appropriate supervision required for patients with epilepsy and muscle spasticity
- Use with caution in patients with peptic ulcers, cerebrovascular diseases, or hepatic, renal or respiratory failure
- Careful monitoring of respiratory and cardiovascular function essential
- Neurogenic disturbances affecting emptying of the bladder may improve, but caution needed in patients with preexisting sphincter hypertonia
- No significant benefit in stroke patients, poor tolerance observed
- Periodic laboratory tests recommended for patients with hepatic diseases or diabetes mellitus
Overdose Effects
- Gastric lavage is important in case of severe overdose
