Indications
- spasticity resulting from multiple sclerosis
- flexor spasms and concomitant pain
- clonus and muscular rigidity
- skeletal muscle spasm resulting from rheumatic disorders
- spinal cord injuries and other spinal cord diseases
- cerebrovascular accidents or neoplastic or degenerative brain disease
Pharmacology
- inhibits monosynaptic and polysynaptic reflexes at the spinal level
- stimulates the GABAB receptors
- inhibits release of glutamate and aspartate
- exerts antinociceptive effect
- produces CNS depression
Dosage & Administration
- 5 mg 3 times daily for adults & children over 10 years
- gradually increased; max. 100 mg daily
- initial treatment with 2.5 mg for children under 10 years
- increased according to requirement based on age
- maintenance doses for different age groups
Interaction
- increased sedation when taken with CNS agents, alcohol, or synthetic opiates
- potentiation effects with tricyclic antidepressants
- reports of mental confusion, hallucinations, headaches, and nausea with concomitant use of Parkinson's medication
- increased CNS depressant effects when used with MAO inhibitors
- caution when used with magnesium sulfate or other neuromuscular blocking agents

Pregnancy & Lactation
- Pregnancy category B3
- crosses placental barrier
- use only when benefits outweigh hazards
- excreted in breast milk
- evidence suggests minimal effect on infants
Precautions & Warnings
- lower doses for impaired renal function
- caution for patients with psychotic disorders, schizophrenia, depressive or manic disorders
- appropriate supervision for patients with epilepsy and muscle spasticity
- caution in patients with peptic ulcers, cerebrovascular diseases, hepatic, renal, or respiratory failure
- monitoring of respiratory and cardiovascular function needed
- improvement in neurogenic disturbances affecting bladder emptying
- poor tolerance in stroke patients
- appropriate laboratory tests for hepatic diseases or diabetes mellitus
Overdose Effects
- gastric lavage in case of severe overdose
