Indications
- Spasticity resulting from multiple sclerosis
- Flexor spasms and concomitant pain
- Clonus and muscular rigidity
- Skeletal muscle spasm resulting from rheumatic disorders
- Spinal cord injuries and other spinal cord diseases
- Cerebrovascular accidents or neoplastic or degenerative brain disease
Pharmacology
Baclofen inhibits both monosynaptic and polysynaptic reflexes at the spinal level by stimulating the GABA B receptors, which inhibits the release of glutamate and aspartate. It may also act at intraspinal sites producing CNS depression. Baclofen also exerts an antinociceptive effect.
Dosage & Administration
- 5 mg 3 times daily for adults & children over 10 years
- Gradually increased dosage with a max of 100 mg daily
- Dosage for children under 10 years based on age range
Interaction
- Increased sedation may occur with CNS agents, alcohol, or synthetic opiates
- Combined treatment with antihypertensives
- Concomitant administration with tricyclic antidepressants may potentiate effects
- Reports of mental confusion, hallucinations, headaches, nausea and agitation with Parkinson's treatment
- Concurrent use of MAO inhibitors
- Caution with magnesium sulfate or other neuromuscular blocking agents

Pregnancy & Lactation
Pregnancy category B3. Safe use not established during pregnancy. Excreted into breast milk with negligible effects on the infant expected.
Precautions & Warnings
- Lower doses for impaired renal function or chronic haemodialysis
- Cautious treatment for patients with psychotic disorders
- Cautious use in patients with epilepsy
- Caution in patients with peptic ulcers, cerebrovascular diseases, or hepatic, renal or respiratory failure
- Monitoring respiratory and cardiovascular function
- Neurogenic disturbances affecting bladder emptying
- Limited benefit for stroke patients
- Regular laboratory tests for hepatic diseases or diabetes mellitus
Overdose Effects
Gastric lavage is important in case of severe overdose
