Indications
- Gastric and duodenal ulcer
- NSAID-associated duodenal and gastric ulcer
- Prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcer
- Gastro-esophageal reflux disease
- Long-term management of acid reflux disease
- Acid-related dyspepsia
- Severe ulcerating reflux esophagitis
- Prophylaxis of acid aspiration during general anesthesia
- Zollinger-Ellison syndrome
- Helicobacter pylori-induced peptic ulcer
Pharmacology
- Inhibitor of gastric acid secretion
- Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
- Onset of antisecretory effect occurs within one hour
- Maximum effect occurs within two hours
- Inhibition of secretion lasts up to 72 hours
- Gradual return of secretory activity over 3 to 5 days after discontinuation
Dosage
- 20 mg once daily for benign gastric and duodenal ulcer
- 40 mg daily for severe or recurrent cases of gastric ulceration
- 10-20 mg daily for long-term management of acid reflux disease
- 40 mg for prophylaxis of acid aspiration
- 60 mg once daily for Zollinger-Ellison syndrome
- 20 mg twice daily for Helicobacter pylori eradication regimen
Administration
- Omeprazole is for intravenous administration only
- Injection should be given slowly over a period of at least 2 to 5 minutes
- IV infusion should be given over a period of 20-30 minutes or more
Interaction
- Decreased intragastric acidity affecting ketoconazole absorption
- Metabolism through cytochrome P450 affecting diazepam, phenytoin, and warfarin elimination
- Plasma concentration increase during concomitant administration with clarithromycin
- Interactions with various drugs including alcohol, food, piroxicam, diclofenac, and naproxen
Contraindications
- Hypersensitivity to omeprazole
- Suspicion of gastric ulcer malignancy without exclusion
Side Effects
- Skin rash, urticaria, and pruritus
- Photosensitivity, bullous eruption, and erythema multiforme
- Diarrhoea, headache, constipation, and nausea/vomiting
- Dry mouth, stomatitis, candidiasis, and paraesthesia
- Dizziness, light headedness, and feeling faint
- Reversible mental confusion, agitation, and depression
- Arthritic and myalgic symptoms
- Increased liver enzymes
Pregnancy & Lactation
- No adverse effects on pregnancy or fetus/newborn child indicated by epidemiological studies
- Can be used during pregnancy
- Discontinue breastfeeding if essential to use omeprazole
Precautions & Warnings
- Avoid concomitant use with clopidogrel
- Possible association with increased risk for osteoporosis-related fractures
- Possible development of atrophic gastritis
- Concomitant use with methotrexate may lead to methotrexate toxicities
