Indications
- Eczema treatment for children and adults
- Treatment for infantile and discoid eczema
- Management of prurigo nodularis
- Psoriasis treatment (excluding wide spread plaque psoriasis)
- Management of neurodermatoses including lichen simplex and lichen planus
- Treatment for seborrheic dermatitis
- Management of contact sensitivity reaction
- Adjunct therapy to systemic steroid therapy in generalized erythroderma
Pharmacology
- Betamethasone Valerate BP as synthetic adrenocorticosteroid
- Glucocorticoid in nature
- Analog of prednisolone with slight mineral corticosteroid activity
- Anti-inflammatory, antipruritic, and vasoconstrictive activity suitable for dermatological use
- Permeates skin and absorption is increased by skin inflammation or disease process
- Percutaneous absorption substantially increased by occlusive dressings
Dosage & Administration
- Sparing application to affected area two or three times daily
- Once improvement occurs, reduced to twice daily or less
- Usual maximum therapy duration of three weeks
Interaction
- No significant reported drug interactions with Besone Cream/Ointment
Contraindications
- Hypersensitivity to any of the ingredients
- Contraindicated for rosacea and acne vulgaris
- Avoid in perioral dermatitis, perianal, and genital pruritus
- Avoid in viral skin infections like herpes simplex and chicken pox
- Primary infected skin lesions caused by fungi or bacteria
- Not for dermatoses in children under one year of age
Side Effects
- Common local adverse reactions with high doses and long-term use
- More common with occlusive dressings: dryness, itching, burning, skin thinning, local irritation
- Features of hypercorticolism, telangiectasia, striae, skin atrophy, hypertrichosis
- Change in pigmentation, secondary infection, perioral dermatitis, allergic contact dermatitis
- Maceration of the skin, acneform eruption, exacerbation of symptoms
Pregnancy & Lactation
- Extensive use not recommended during pregnancy
Precautions & Warnings
- Stop application if signs of hypersensitivity appear
- Avoid long-term continuous therapy, especially in children as adrenal suppression may occur
- Systemic absorption may occur with extensive treatment, leading to symptoms of hypercorticolism
- Atrophic changes may occur on the face or other areas after prolonged treatment
- Ensure the preparation does not enter the eye if applied to eyelids to prevent glaucoma
