Indications
- Gastric and duodenal ulcer
- NSAID-associated duodenal and gastric ulcer
- Prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcer
- Gastro-esophageal reflux disease
- Long-term management of acid reflux disease
- Acid-related dyspepsia
- Severe ulcerating reflux esophagitis
- Prophylaxis of acid aspiration during general anesthesia
- Zollinger-Ellison syndrome
- Helicobacter pylori-induced peptic ulcer
Pharmacology
Inhibitor of gastric acid secretion by blocking hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system in the gastric parietal cell
Dosage
- 20 mg once daily for benign gastric and duodenal ulcer
- 10-20 mg once daily for acid-related dyspepsia
- 40 mg preceding evening for prophylaxis of acid aspiration
- 20 mg twice daily for Helicobacter pylori eradication regimen in peptic ulcer disease
- 10-40 mg once daily for paediatric severe ulcerating reflux esophagitis
- 40 mg IV injection for prophylaxis of acid aspiration
- 60 mg IV for Zollinger-Ellison syndrome
- 40 mg IV for duodenal ulcer, gastric ulcer, or reflux oesophagitis
Administration
- IV Injection for prophylaxis
- IV Injection for duodenal and gastric ulcer
- IV Injection for Zollinger-Ellison syndrome
- IV Infusion over 20-30 minutes
- IV Infusion over 100 ml saline or 5% Dextrose
Interaction
Decreased intragastric acidity may reduce absorption of ketoconazole, metabolized in the liver through cytochrome P450, may delay elimination of diazepam, phenytoin, and warfarin
Contraindications
Known hypersensitivity to omeprazole, suspicion of malignancy in gastric ulcer
Side Effects
- Skin rash, urticaria, pruritus
- Photosensitivity, bullous eruption, erythema multiforme, angioedema, alopecia
- Diarrhoea, headache
- Constipation, nausea/vomiting, flatulence, abdominal pain
- Dry mouth, stomatitis, candidiasis
- Paraesthesia, dizziness, lightheadedness, feeling faint
- Somnolence, insomnia, vertigo
- Reversible mental confusion, agitation, depression, hallucinations
- Arthritic and myalgic symptoms
- Blurred vision, taste disturbance, peripheral oedema
- Increased sweating, gynaecomastia
- Leucopenia, thrombocytopenia, agranulocytosis, pancytopenia
- Anaphylactic shock, malaise, fever
- Bronchospasm, encephalopathy, hepatitis, hepatic failure
- Increases in liver enzymes
Pregnancy & Lactation
Safe for pregnancy, discontinuation advised for breastfeeding, not established for pediatric patients below 18 years
Precautions & Warnings
- Avoid concomitant use of clopidogrel and omeprazole
- Increased risk of osteoporosis-related fractures with PPI therapy
- Atrophic gastritis in patients treated long-term
- Concomitant use with methotrexate may lead to toxicities
