Indications

  • Treatment of gastric and duodenal ulcer
  • NSAID-associated duodenal and gastric ulcer
  • Prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcer
  • Gastro-esophageal reflux disease
  • Long-term management of acid reflux disease
  • Acid-related dyspepsia
  • Severe ulcerating reflux esophagitis
  • Prophylaxis of acid aspiration during general anesthesia
  • Zollinger-Ellison syndrome
  • Helicobacter pylori-induced peptic ulcer

Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of antisecretory effect within one hour
  • Maximum effect occurring within two hours
  • Inhibition of secretion lasting up to 72 hours
  • Gradual return of secretory activity over 3 to 5 days after discontinuation

Dosage

  • Different doses for various conditions like benign gastric and duodenal ulcer, NSAID-associated duodenal or gastric ulcer, gastro-esophageal reflux disease, long-term management of acid reflux disease, acid-related dyspepsia, prophylaxis of acid aspiration, Zollinger-Ellison syndrome, Helicobacter pylori eradication regimen, and severe ulcerating reflux esophagitis
  • Also includes dosage for pediatric use

Interaction

  • Effects on the absorption of ketoconazole
  • Metabolism through cytochrome P450 affecting elimination of diazepam, phenytoin, and warfarin
  • Monitoring of patients receiving warfarin or phenytoin recommended
  • Plasma concentrations of Losectil and clarithromycin increased during concomitant administration
  • No evidence of interaction with phenacetin, theophylline, caffeine, propranolol, metoprolol, cyclosporin, lidocaine, quinidine, estradiol, amoxycillin, or antacids

Contraindications

  • Known hypersensitivity to omeprazole
  • Suspicion of gastric ulcer without excluding the possibility of malignancy

Side Effects

  • Mild and reversible adverse reactions
  • Reported skin rash, urticaria, pruritus, photosensitivity, bullous eruption, erythema multiforme, angioedema, alopecia, diarrhea, headache, constipation, nausea/vomiting, flatulence, abdominal pain, dry mouth, stomatitis, candidiasis, paraesthesia, dizziness, light headedness, faint feeling, somnolence, insomnia, vertigo, reversible mental confusion, agitation, depression, hallucinations, arthritic and myalgic symptoms, blurred vision, taste disturbance, peripheral edema, increased sweating, gynaecomastia, leucopenia, thrombocytopenia, agranulocytosis, pancytopenia, anaphylactic shock, malaise, fever, bronchospasm, encephalopathy, hepatitis, interstitial nephritis, hepatic failure, increases in liver enzymes

Pregnancy & Lactation

  • No adverse effects observed on pregnancy or the health of the fetus/newborn child based on epidemiological studies
  • Use during pregnancy possible
  • No information on passage into breast milk or effects on the neonate
  • Discontinuation of breast-feeding recommended if essential use of omeprazole

Precautions & Warnings

  • Avoid concomitant use of clopidogrel and omeprazole
  • Possible association of PPI therapy with increased risk for osteoporosis-related fractures
  • Atrophic gastritis noted occasionally in gastric corpus biopsies from patients treated long-term with omeprazole
  • Concomitant use with methotrexate may lead to methotrexate toxicities
