Indications

Pharmacology

  • Additive antihypertensive effects
  • Selective adrenoceptor blocking agent
  • Benzothiadiazine diuretic
  • Affects renal tubular mechanisms
  • Increases excretion of sodium and chloride

Dosage & Administration

  • Effective once-daily doses
  • Antihypertensive effects increase with increasing doses
  • Initial therapy with lowest dose
  • Subsequent titration for maximum recommended dose
  • Substitution for titrated individual components
  • Therapy guided by clinical effect

Interaction

  • Potentiates action of other antihypertensive agents
  • Should not be combined with other beta-blocking agents
  • Closely monitor patients receiving catecholamine-depleting drugs
  • Caution with concurrent use of myocardial depressants or AV conduction inhibitors
  • Increased risk of bradycardia with concomitant use of digitalis glycosides

Contraindications

  • Cardiogenic shock
  • Overt cardiac failure
  • Second or third degree AV block
  • Marked sinus bradycardia
  • Anuria
  • Hypersensitivity to either component

  • Potential for serious adverse reactions in nursing infants

Precautions & Warnings

  • Hyperuricemia or acute gout
  • Warning signs or symptoms of fluid and electrolyte imbalance
  • Hypokalemia risk
  • Gradual withdrawal of therapy
  • Careful observation of patients

Overdose Effects

  • Expected signs with overdose of beta-blocker
  • Lethargy
  • Delirium
  • Coma
  • Convulsions
  • Respiratory arrest
  • Congestive heart failure
  • Bronchospasm
  • Hypoglycemia
  • Acute loss of fluid and electrolytes
  • Signs and symptoms in various body systems
  • Laboratory findings such as hypokalemia, hyponatremia, hypochloremia, alkalosis
