Indications
- Gastric and duodenal ulcer
- NSAID-associated duodenal and gastric ulcer
- As prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcer
- Gastro-esophageal reflux disease
- Long-term management of acid reflux disease
- Acid-related dyspepsia
- Severe ulcerating reflux esophagitis
- Prophylaxis of acid aspiration during general anesthesia
- Zollinger-Ellison syndrome
- Helicobacter pylori-induced peptic ulcer
Pharmacology
Inhibitor of gastric acid secretion
Dosage
- 20 mg once daily for 4 weeks in duodenal ulceration
- 40 mg once daily for severe or recurrent cases
- 40 mg once daily for 8 weeks in refractory gastro-esophageal reflux disease
- 10-20 mg daily for long-term management of acid reflux disease
- 10-20 mg once daily for 2-4 weeks in acid-related dyspepsia
- 40 mg prophylaxis of acid aspiration before surgery
- 60 mg once daily for Zollinger-Ellison syndrome
- 20 mg twice daily with antimicrobial agents for Helicobacter pylori eradication
- Pediatric use: 10-40 mg once daily based on body weight
- IV Injection for various indications
Administration
Must be administered as intravenous injection or infusion
Interaction
- Decreased intragastric acidity affecting absorption of other medications
- Metabolized in the liver through cytochrome P450
- Possible delay in elimination of diazepam, phenytoin, and warfarin
- Monitoring required for patients receiving warfarin or phenytoin
- Increased plasma concentrations during concomitant clarithromycin administration
- No evidence of interaction with several medications including antacids
Contraindications
Known hypersensitivity to omeprazole, suspicion of gastric ulcer as malignancy
Side Effects
- Mild and reversible adverse reactions
- Skin rash, urticaria and pruritus
- Photosensitivity, bullous eruption, erythema multiforme
- Diarrhoea, headache, constipation, nausea/vomiting
- Dizziness, somnolence, insomnia, vertigo
- Reversible mental confusion and agitation
- Arthritic and myalgic symptoms
- Possible increases in liver enzymes
Pregnancy & Lactation
Can be used during pregnancy, information on passage into breast milk is not available
Precautions & Warnings
- Avoid concomitant use with clopidogrel
- Possible association with increased risk for osteoporosis-related fractures
- Atrophic gastritis noted in long-term treatment
- Concomitant use with methotrexate may lead to toxicities
