Indications

Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of antisecretory effect within one hour
  • Maximum effect within two hours
  • Inhibition of secretion lasts up to 72 hours

Dosage

  • 20 mg once daily for benign gastric and duodenal ulcer
  • 40 mg daily in severe or recurrent cases
  • 10-20 mg daily for long-term management of acid reflux disease
  • 10-20 mg once daily for acid-related dyspepsia
  • 40 mg on the preceding evening for prophylaxis of acid aspiration
  • 60 mg once daily for Zollinger-Ellison syndrome
  • 20 mg twice daily for Helicobacter pylori eradication regimen in peptic ulcer disease
  • 10-20 mg once daily for paeditaric use in severe ulcerating reflux esophagitis

Administration

  • IV injection for prophylaxis of acid aspiration
  • IV infusion over a period of 20-30 minutes or more

Interaction

  • Decreased intragastric acidity may reduce absorption of ketoconazole
  • Metabolized in the liver through cytochrome P450
  • Can delay the elimination of diazepam, phenytoin, and warfarin
  • Plasma concentrations increased during concomitant administration with clarithromycin
  • No evidence of interaction with phenacetin, theophylline, caffeine, and more

Contraindications

  • Known hypersensitivity to omeprazole
  • Suspicion of gastric ulcer, excluding malignancy before treatment

Side Effects

  • Skin rash, urticaria, and pruritus
  • Diarrhoea and headache
  • Constipation, nausea/vomiting, flatulence, and abdominal pain
  • Dizziness, light headedness, and feeling faint
  • Reversible mental confusion, agitation, depression, and hallucinations
  • Increases in liver enzymes

Pregnancy & Lactation

  • No adverse effects on pregnancy or on the health of the fetus/newborn child

Precautions & Warnings

  • Avoid concomitant use of clopidogrel and omeprazole
  • Observational studies suggest PPI therapy may be associated with an increased risk for osteoporosis-related fractures
