Indications

  • Gastric and duodenal ulcer
  • NSAID-associated duodenal and gastric ulcer
  • Prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcer
  • Gastro-esophageal reflux disease
  • Long-term management of acid reflux disease
  • Acid-related dyspepsia
  • Severe ulcerating reflux esophagitis
  • Prophylaxis of acid aspiration during general anesthesia
  • Zollinger-Ellison syndrome
  • Helicobacter pylori-induced peptic ulcer

Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of antisecretory effect within one hour
  • Maximum effect occurring within two hours
  • Inhibition of secretion lasts up to 72 hours
  • Secretory activity returns gradually over 3 to 5 days after discontinuation

Administration

  • Direction for use of IV Injection
  • Omeprazole IV injection for intravenous administration only
  • IV infusion over a period of 20-30 minutes or more
  • Reconstituted solution should not be mixed with any other drug

Interaction

  • Decreased intragastric acidity affecting the absorption of ketoconazole
  • Delayed elimination of diazepam, phenytoin, and warfarin
  • Monitoring of patients receiving warfarin or phenytoin recommended
  • Concomitant treatment with Omenix did not change blood concentration of phenytoin
  • Concomitant treatment with Omenix did not change coagulation time in patients on continuous treatment with warfarin
  • Plasma concentrations of Omeprazole and clarithromycin increased during concomitant administration
  • No evidence of interaction with various drugs including phenacetin, theophylline, caffeine, propranolol, metoprolol, cyclosporin, lidocaine, quinidine, estradiol, amoxicillin, antacids, piroxicam, diclofenac, naproxen, alcohol, or food
  • Simultaneous treatment with Omenix and digoxin led to a 10% increase in the bioavailability of digoxin

Contraindications

  • Known hypersensitivity to Omeprazole
  • Suspected gastric ulcer with possibility of malignancy

Side Effects

  • Mild and reversible adverse reactions
  • Skin rash, urticaria, and pruritus reported
  • Photosensitivity, bullous eruption, erythema multiforme, angioedema, and alopecia reported in isolated cases
  • Diarrhoea and headache reported
  • Other gastrointestinal reactions including constipation, nausea/vomiting, flatulence, and abdominal pain reported
  • Dry mouth, stomatitis, and candidiasis reported as isolated cases
  • Paraesthesia, dizziness, light headedness, and feeling faint associated with treatment
  • Somnolence, insomnia, and vertigo reported
  • Reversible mental confusion, agitation, depression, and hallucinations reported predominantly in severely ill patients
  • Arthritic and myalgic symptoms reported
  • Various isolated cases reported including blurred vision, taste disturbance, peripheral oedema, increased sweating, gynaecomastia, blood disorders, shock, malaise, fever, bronchospasm, encephalopathy, hepatitis, interstitial nephritis, hepatic failure, and increases in liver enzymes

Pregnancy & Lactation

  • No adverse effects on pregnancy or on the health of the fetus/newborn child reported
  • Omeprazole can be used during pregnancy
  • No information available on the passage of Omeprazole into breast milk or its effects on the neonate
  • Breastfeeding should be discontinued if the use of Omeprazole is considered essential
  • Safety and effectiveness not established in pediatric patients less than 18 years of age

Precautions & Warnings

  • Avoid concomitant use of clopidogrel and Omeprazole
  • Proton pump inhibitor (PPI) therapy may be associated with an increased risk for osteoporosis-related fractures
  • Atrophic gastritis noted occasionally in patients treated long-term with Omeprazole
  • Concomitant use of PPIs with methotrexate may lead to methotrexate toxicities
