Indications
- Treatment of mild, moderate or severe heart failure
- Treatment in conjunction with digitalis, diuretics, and ACE inhibitor
- Reduction of cardiovascular death
- Reduction of cardiovascular hospitalization
- Usable in patients unable to tolerate an ACE inhibitor
- Usable in patients not receiving digitalis, hydralazine, or nitrate therapy
Pharmacology
- Non-selective antagonism of β-adrenergic receptors
- Alpha-adrenergic antagonistic activity
- Antiproliferative properties
- Scavenger of reactive free oxidant radicals
- Used in treatment of hypertension, angina pectoris, and congestive heart failure
Dosage & Administration
- Initial dose of 12.5 mg once daily for hypertension
- Usual dose of 25 mg once daily for hypertension
- Initial dose of 12.5 mg twice daily for angina pectoris
- Recommended dosage of 25 mg twice daily for angina pectoris
- Initial dose of 3.125 mg twice daily for heart failure
- Dose may be increased at intervals of at least 2 weeks
Interaction
- Increased monitoring of digoxin recommended
- Rifampin may result in decreased Cmax and AUC
- No alteration in in vitro plasma protein binding of warfarin
- Potentiation of the pressor reaction with clonidine withdrawal
Contraindications
- Decompensated heart failure requiring intravenous inotropic therapy
- Bronchial asthma or related bronchospastic conditions
- Second or third-degree AV block
- Sick sinus syndrome without a permanent pacemaker
- Cardiogenic shock
- Severe bradycardia

Precautions & Warnings
- Caution in hepatic impairment
- Monitor clinical status for 2-3 hours after initiation
