Indications
- Gastric and duodenal ulcer
- NSAID-associated duodenal and gastric ulcer
- Gastro-esophageal reflux disease
- Acid-related dyspepsia
- Severe ulcerating reflux esophagitis
- Prophylaxis of acid aspiration during general anesthesia
- Zollinger-Ellison syndrome
- Helicobacter pylori-induced peptic ulcer
Pharmacology
Inhibitor of gastric acid secretion, onset of antisecretory effect within one hour, maximum effect within two hours, inhibition lasts up to 72 hours
Dosage
- 20 mg once daily for benign gastric and duodenal ulcer
- 40 mg once daily for prophylaxis of acid aspiration during general anesthesia
- 20 mg twice daily for Helicobacter pylori eradication regimen in peptic ulcer disease
- 10-20 mg daily for long-term management of acid reflux disease
Administration
IV Injection for prophylaxis of acid aspiration should be given slowly over a period of 5 minutes, IV infusion should be given over a period of 20-30 minutes or more
Interactions
- Decreased intragastric acidity affecting absorption of ketoconazole
- Metabolism through cytochrome P450 affecting drugs like diazepam, phenytoin, and warfarin
- Increased plasma concentrations with clarithromycin during H. pylori eradication
- No evidence of interaction with alcohol or food
Contraindications
Known hypersensitivity
Side Effects
- Mild and reversible adverse reactions
- Skin rash, urticaria, and pruritus reported
- Diarrhea and headache in some cases
- Gastrointestinal reactions including constipation, nausea, and flatulence
- Dizziness, somnolence, insomnia, and vertigo reported
- Reversible mental confusion, agitation, and depression in severely ill patients
Pregnancy & Lactation
Results from prospective epidemiological studies indicate no adverse effects on pregnancy or fetus/newborn child, breast-feeding discontinuation if use is essential
Precautions & Warnings
- Avoid concomitant use of clopidogrel
- Increased risk for osteoporosis-related fractures in observational studies
- Atrophic gastritis noted occasionally in long-term use
- Concomitant use with methotrexate may lead to toxicities
