Indications
- Gastric and duodenal ulcer
- NSAID-associated duodenal and gastric ulcer
- Prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcer
- Gastro-esophageal reflux disease
- Long-term management of acid reflux disease
- Acid-related dyspepsia
- Severe ulcerating reflux esophagitis
- Prophylaxis of acid aspiration during general anesthesia
- Zollinger-Ellison syndrome
- Helicobacter pylori-induced peptic ulcer
Pharmacology
- Inhibitor of gastric acid secretion
- Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
- Onset of the antisecretory effect within one hour
- Maximum effect occurring within two hours
- Inhibition of secretion lasting up to 72 hours
Dosage
- 20 mg once daily for 4 weeks in duodenal ulceration
- 8 weeks in gastric ulceration
- 40 mg daily in severe or recurrent cases
- Prevention of duodenal ulcer relapse with 10-20 mg daily
- 20 mg once daily for 4 weeks in NSAID-associated duodenal or gastric ulcer
- Gastro-esophageal reflux disease treatment with 20 mg once daily for 4-8 weeks
- Long-term management of acid reflux disease with 10-20 mg daily
- Maintenance dose for acid-related dyspepsia is 10-20 mg once daily for 2-4 weeks
- Prophylaxis of acid aspiration with 40 mg on the preceding evening and 2-6 hours before surgery
- Zollinger-Ellison syndrome treatment starting with 60 mg once daily, usual range 20-120 mg daily
- Paediatric use in severe ulcerating reflux esophagitis with 10-40 mg once daily for 4-12 weeks
Administration
- IV Injection for prophylaxis of acid aspiration with 40 mg given slowly over a period of 5 minutes
- IV Infusion as an intravenous infusion over a period of 20-30 minutes or more
Interaction
- Reduced absorption of ketoconazole during Ometac treatment
- Metabolized in the liver through cytochrome P450
- Possible delay in the elimination of diazepam, phenytoin and warfarin
- Increased plasma concentrations of Ometac and clarithromycin during concomitant administration
- No evidence of interaction with phenacetin, theophylline, caffeine, propranolol, metoprolol, cyclosporin, lidocaine, quinidine, estradiol, amoxycillin or antacids
- Increased bioavailability of digoxin during simultaneous treatment with Ometac
Contraindications
- Known hypersensitivity to omeprazole
- Suspicion of gastric ulcer malignancy
Side Effects
- Skin rash, urticaria and pruritus
- Photosensitivity, bullous eruption, erythema multiforme, angioedema and alopecia
- Diarrhoea and headache
- Gastrointestinal reactions including constipation, nausea/vomiting, flatulence, and abdominal pain
- Dry mouth, stomatitis and candidiasis
- Paraesthesia, dizziness, and lightheadedness
- Somnolence, insomnia, and vertigo
- Reversible mental confusion, agitation, depression, and hallucinations
- Arthritic and myalgic symptoms
- Blurred vision, taste disturbance, peripheral oedema, and increased sweating
- Gynaecomastia, leucopenia, thrombocytopenia, agranulocytosis, and pancytopenia
- Anaphylactic shock, malaise, fever, bronchospasm, and encephalopathy
- Hepatitis with or without jaundice, interstitial nephritis, and hepatic failure
- Increases in liver enzymes
Pregnancy & Lactation
- No adverse effects of Omeprazole on pregnancy or the fetus/newborn child from prospective epidemiological studies
- Omeprazole can be used during pregnancy
- No information available on the passage of Omeprazole into breast milk or its effects on the neonate
- Discontinuation of breastfeeding if essential use of Omeprazole
Precautions & Warnings
- Avoid concomitant use of clopidogrel and Ometac
- Possible association of PPI therapy with an increased risk for osteoporosis-related fractures
- Atrophic gastritis occasionally noted in patients treated long-term with Ometac
- Concomitant use of PPIs with methotrexate may lead to methotrexate toxicities
