Indications
- Gastric and duodenal ulcer
- NSAID-associated duodenal and gastric ulcer
- Prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcer
- Gastro-esophageal reflux disease
- Long-term management of acid reflux disease
- Acid-related dyspepsia
- Severe ulcerating reflux esophagitis
- Prophylaxis of acid aspiration during general anesthesia
- Zollinger-Ellison syndrome
- Helicobacter pylori-induced peptic ulcer
Pharmacology
Inhibitor of gastric acid secretion by blocking hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system in the gastric parietal cell
Dosage
- Oral:
- 20 mg once daily for 4-8 weeks in various conditions
- 10-20 mg daily for long-term management of acid reflux disease
- 40 mg on the preceding evening and 2-6 hours before surgery for prophylaxis of acid aspiration
- Dose adjustments for Zollinger-Ellison syndrome and pediatric use
- IV Injection:
- Prophylaxis of acid aspiration
- Duodenal ulcer, gastric ulcer, or reflux oesophagitis
- Zollinger-Ellison syndrome
Administration
- IV Injection: Given slowly over a period of 5 minutes as an intravenous injection
- IV Infusion: Given as an intravenous infusion over a period of 20-30 minutes or more, dissolved in saline or 5% Dextrose
Interaction
- Decreased intragastric acidity affecting the absorption of ketoconazole
- Metabolism through cytochrome P450 affecting the elimination of certain drugs
- Increased plasma concentrations during concomitant administration with other drugs
Contraindications
Known hypersensitivity
Side Effects
- Mild and reversible adverse reactions
- Skin rash, urticaria, pruritus, photosensitivity, bullous eruption
- Diarrhoea, headache, constipation, nausea/vomiting
- Dry mouth, stomatitis, candidiasis, paraesthesia
- Dizziness, light headedness, somnolence, insomnia, vertigo
- Reversible mental confusion, agitation, depression, hallucinations
- Arthritic and myalgic symptoms, blurred vision, taste disturbance
- Increased sweating, gynaecomastia, leucopenia, thrombocytopenia
- Agranulocytosis, pancytopenia, anaphylactic shock, malaise, fever
- Bronchospasm, encephalopathy, hepatitis with or without jaundice
- Interstitial nephritis, hepatic failure, increases in liver enzymes observed
Pregnancy & Lactation
- No adverse effects on pregnancy or on the health of the fetus/newborn child
- Usage during pregnancy considered safe
- No information available on the passage into breast milk or its effects on the neonate
- Breastfeeding should be discontinued if essential
- Safety and effectiveness not established in pediatric patients less than 18 years of age
Precautions & Warnings
- Avoid concomitant use with clopidogrel
- Increased risk for osteoporosis-related fractures
- Atrophic gastritis noted occasionally in long-term use
- Concomitant use with methotrexate may lead to toxicities
