Indications

  • Gastric and duodenal ulcer
  • NSAID-associated duodenal and gastric ulcer
  • Gastro-esophageal reflux disease
  • Long-term management of acid reflux disease
  • Severe ulcerating reflux esophagitis
  • Prophylaxis of acid aspiration during general anesthesia
  • Zollinger-Ellison syndrome
  • Helicobacter pylori-induced peptic ulcer

Pharmacology

Inhibitor of gastric acid secretion; blocks hydrogen-potassium-adenosine triphosphatase (H/K ATPase) enzyme system

Dosage

  • 20 mg once daily for 4 weeks in duodenal ulceration
  • 40 mg once daily in severe or recurrent cases
  • 10-20 mg daily for acid-related dyspepsia
  • 40 mg on the preceding evening for prophylaxis of acid aspiration

Administration

  • Omeprazole IV injection should be given as a slow intravenous injection
  • Omeprazole IV infusion should be given as an intravenous infusion over a period of 20-30 minutes or more

Interaction

  • Decreased intragastric acidity may reduce absorption of ketoconazole
  • Metabolized in the liver through cytochrome P450
  • Plasma concentrations of Omeprazole and clarithromycin are increased during concomitant administration

Contraindications

Contraindicated in patients with known hypersensitivity to it. The possibility of malignancy should be excluded before treatment if gastric ulcer is suspected.

  • Headache
  • Nausea/vomiting
  • Dizziness and light-headedness
  • Somnolence and insomnia
  • Reversible mental confusion
  • Arthritic and myalgic symptoms
  • Increases in liver enzymes

Pregnancy & Lactation

Results from epidemiological studies indicate no adverse effects on pregnancy or on the health of the fetus/newborn child. Breast-feeding should be discontinued if the use of Omeprazole is considered essential.

Precautions & Warnings

  • Avoid concomitant use of clopidogrel and Omizit
  • PPI therapy may be associated with an increased risk for osteoporosis-related fractures
  • Atrophic gastritis has been noted occasionally
