Description
- OPmax is a medicine used for the treatment of gastric and duodenal ulcers, NSAID-associated ulcers, gastro-esophageal reflux disease, acid-related dyspepsia, and other related conditions. It is also used for prophylaxis of acid aspiration during general anesthesia, Zollinger-Ellison syndrome, and Helicobacter pylori-induced peptic ulcers.
Dosage instructions
- 20 mg once daily for 4-8 weeks for gastric and duodenal ulcer
- 10-20 mg daily for long-term management of acid reflux disease
- 40 mg on the preceding evening and 2-6 hours before surgery for prophylaxis of acid aspiration
- 60-120 mg daily for Zollinger-Ellison syndrome
- 20 mg twice daily with antimicrobial agents for Helicobacter pylori eradication regimen
- 10-40 mg daily for severe ulcerating reflux esophagitis in pediatric use
Administration
- IV injection should be given slowly over a period of 5 minutes as prophylaxis of acid aspiration
- IV infusion should be given over a period of 20-30 minutes or more
- Reconstituted solution should be used within 4-12 hours based on the diluent used
Pharmacology
- Omeprazole is an inhibitor of gastric acid secretion that acts by blocking H+/K+ ATPase enzyme system in the gastric parietal cell.
Interaction
- Decreased intragastric acidity may reduce the absorption of ketoconazole during OPmax treatment
- OPmax can delay the elimination of diazepam, phenytoin, and warfarin due to its metabolism in the liver through cytochrome P450
- Plasma concentrations of OPmax and clarithromycin are increased during concomitant administration
Contraindications
- Known hypersensitivity to Omeprazole
- Suspicion of gastric ulcer with possibility of malignancy
Side effects
- Skin rash, urticaria, and pruritus
- Photosensitivity, bullous eruption, and erythema multiforme
- Diarrhea, headache, constipation, and nausea/vomiting
- Dry mouth, stomatitis, and candidiasis
Pregnancy lactation
- No adverse effects of Omeprazole on pregnancy or the health of the fetus/newborn child based on epidemiological studies
- Discontinue breastfeeding if essential to use Omeprazole
Precautions warnings
- Avoid concomitant use of clopidogrel and OPmax
- PPI therapy may be associated with an increased risk for osteoporosis-related fractures
- Atrophic gastritis noted occasionally in long-term OPmax treatment
- Concomitant use of PPIs with methotrexate may lead to toxicities
