Indications
- Gastric and duodenal ulcer
- NSAID-associated duodenal and gastric ulcer
- Gastro-esophageal reflux disease
- Acid-related dyspepsia
- Severe ulcerating reflux esophagitis
- Prophylaxis of acid aspiration during general anesthesia
- Zollinger-Ellison syndrome
- Helicobacter pylori-induced peptic ulcer
- Long-term management of acid reflux disease
Pharmacology
- Inhibitor of gastric acid secretion
- Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
- Onset of antisecretory effect within one hour
- Maximum effect within two hours
- Inhibition of secretion lasts up to 72 hours
Dosage
- Oral administration
- 20 mg once daily for duodenal ulceration
- 8 weeks in gastric ulceration
- 40 mg daily for severe or recurrent cases
- 10-20 mg daily for prevention of relapse in duodenal ulcer
- 20 mg for 4 weeks in gastro-esophageal reflux disease
- 40 mg for 8 weeks in refractory cases of gastro-esophageal reflux disease
- 10-20 mg daily for long-term management of acid reflux disease
Administration
- IV injection for prophylaxis of acid aspiration
- IV infusion over 20-30 minutes
- Intravenous administration only
- Slow intravenous injection
- Reconstitution with water for injection or saline for infusion
- Use within specified time after reconstitution
Interaction
- Decreased intragastric acidity affects ketoconazole absorption
- Metabolized in liver through cytochrome P450
- Can delay elimination of diazepam, phenytoin, and warfarin
- Potential interactions with clarithromycin, digoxin, and other drugs
Contraindications
- Hypersensitivity to omeprazole
- Exclusion of malignancy before treatment
Side Effects
- Mild and reversible adverse reactions
- Skin rash, urticaria, and pruritus
- Photosensitivity, bullous eruption, and erythema multiforme
- Diarrhoea and headache
- Gastrointestinal reactions including constipation and nausea/vomiting
- Dizziness, light headedness, and somnolence
- Reversible mental confusion and myalgic symptoms
- Increases in liver enzymes
Pregnancy & Lactation
- No adverse effects on pregnancy or fetus/newborn child reported in studies
- Use during pregnancy considered safe
- No information on passage into breast milk or effects on neonate
- Breastfeeding should be discontinued if essential use is considered
- Safety and effectiveness not established in pediatric patients
Precautions & Warnings
- Avoid concomitant use with clopidogrel
- Possible association with increased risk for osteoporosis-related fractures
- Atrophic gastritis noted in long-term treated patients
- Potential methotrexate toxicities with concomitant use
