Indications

Pharmacology

  • Active form: Calcitriol
  • Binding to Vitamin D receptors (VDRs)
  • Absorbed in the small intestine
  • Metabolised to 25-hydroxy Vitamin D3
  • Half-life of about 50 days
  • Second hydroxylation in the kidney
  • Increases the absorption of calcium
  • Stored in fat and muscle
  • Elimination via faeces and urine

  • Adult dosage - for Vitamin D3 deficiency and maintenance therapy
  • Children dosage - for Vitamin D3 deficiency and prevention
  • Dosage forms: capsule, film-coated tablet, oroflash/chewable tablets, syrup, injection
  • Daily dosage for different forms
  • Dosage for specific therapy for osteoporosis
  • Dosage for different age groups and specific conditions

Interaction

  • Drugs causing Vitamin D3 deficiencies
  • Drugs interfering with absorption and metabolism of Vitamin D3
  • Drugs increasing the need for Vitamin D3

Contraindications

  • Contraindicated in diseases associated with hypercalcaemia
  • Contraindicated in patients with known hypersensitivity to Vitamin D3
  • Contraindicated if evidence of Vitamin D3 toxicity

Side Effects

  • Symptoms: anorexia, lassitude, nausea & vomiting, diarrhea, constipation, weight loss, polyuria, sweating, headache, thirst, vertigo
  • Raised concentrations of calcium and phosphate in plasma and urine

Pregnancy & Lactation

  • Safe use up to 4000 IU during pregnancy
  • Excreted in breast milk
  • Need for higher dose during deficiency
  • Exogenous vitamin D3 dosage for breast-fed child

Precautions & Warnings

  • Monitoring plasma-calcium concentration
  • Consultation for patients using Digoxin and Thiazide Diuretics
  • Consultation for patients with liver or kidney disease, primary hyperthyroidism, lymphoma, tuberculosis, and granulomatous disease

Use in Special Populations

  • Safety & efficacy not established in children under 12 years

Overdose Effects

  • Hypervitaminosis D
