Medicine Details

Indications

  • Acute bacterial exacerbation of chronic bronchitis in adults
  • Acute maxillary sinusitis
  • Community-acquired pneumonia
  • Pharyngitis or tonsillitis
  • Uncomplicated skin and skin structure infections
  • Acute otitis media in pediatric patients
  • Treatment and prophylaxis of disseminated mycobacterial infections
  • Helicobacter pylori infection and duodenal ulcer disease in adults

Pharmacology

  • Inhibits microsomal protein synthesis
  • Binds to the donor site on the 50S subunit of the bacterial ribosome
  • Active against most Gram-positive bacteria, Chlamydia, some Gram-negative bacteria, and Mycoplasmas
  • Twice the activity of Erythromycin against H. influenzae
  • More acid-stable than Erythromycin

Dosage & Administration

  • Different dosages for specific conditions in adults
  • Combination dosing regimens for H. pylori infection
  • Recommended daily dosage for children
  • Usage in patients with renal and hepatic impairment

Interaction

  • Contraindicated with HMG-CoA reductase inhibitors, gastroprokinetic agents, ergot alkaloids, antipsychotics, anti-gout agents
  • Not recommended with antiarrhythmics drugs

Contraindications

  • Known hypersensitivity to clarithromycin or other macrolide antibacterial drugs
  • History of cholestatic jaundice or hepatic dysfunction associated with prior use of clarithromycin

Pregnancy & Lactation

Precautions & Warnings

  • Discontinue if severe acute hypersensitivity reactions occur
  • Avoid in patients with known QT prolongation or receiving drugs known to prolong the QT interval
  • Discontinue if signs and symptoms of hepatitis occur
  • Increased risk of all-cause mortality in patients with coronary artery disease
  • Evaluation for Clostridium difficile associated diarrhea if diarrhea occurs
  • Exacerbation of myasthenia gravis in patients receiving Clarithromycin therapy

Use in Special Populations

  • Safety and effectiveness in pediatric patients
  • No safety and effectiveness in MAC patients under the age of 20 months
  • Safety and effectiveness not established in pediatric patients under 6 months of age

Overdose Effects

  • Expected gastro-intestinal symptoms with large ingestions
  • Altered mental status
  • Paranoid behaviour
  • Hypokalemia
  • Hypoxemia
  • Prompt elimination of unabsorbed drug and supportive measures for adverse reactions
