Indications

  • Gastric and duodenal ulcer

Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocker of hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of antisecretory effect within one hour
  • Maximum effect within two hours
  • Inhibition of secretion lasts up to 72 hours

Dosage

  • Benign gastric and duodenal ulcer: 20 mg once daily for 4 weeks
  • NSAID-associated duodenal or gastric ulcer: 20 mg once daily for 4 weeks
  • Gastro-esophageal reflux disease: 20 mg once daily for 4 weeks
  • Long-term management of acid reflux disease: 10-20 mg daily
  • Prophylaxis of acid aspiration: 40 mg on the preceding evening, then 40 mg 2-6 hours before surgery
  • Zollinger-Ellison syndrome: Initially 60 mg once daily
  • Paediatric use in severe ulcerating reflux esophagitis: If body-weight 10-20 kg, 10-20 mg once daily for 4-12 weeks

Administration

  • For intravenous administration only
  • IV Injection: Given slowly over a period of 5 minutes
  • IV Infusion: Given over a period of 20-30 minutes or more

Interaction

  • Decreased intragastric acidity affecting the absorption of ketoconazole
  • Metabolized in the liver through cytochrome P450
  • Can delay the elimination of diazepam, phenytoin and warfarin
  • Plasma concentrations increased during concomitant administration with other drugs

Contraindications

  • Known hypersensitivity to omeprazole
  • Suspicion of gastric ulcer with possibility of malignancy

Side Effects

  • Skin rash, urticaria, pruritus
  • Photosensitivity, bullous eruption, erythema multiforme
  • Diarrhoea, headache, constipation
  • Nausea/vomiting, flatulence, abdominal pain
  • Dry mouth, stomatitis, candidiasis
  • Paraesthesia, dizziness, light headedness
  • Somnolence, insomnia, vertigo
  • Mental confusion, agitation, depression
  • Arthritic and myalgic symptoms
  • Blurred vision, taste disturbance, peripheral oedema
  • Increased sweating, gynaecomastia, leucopenia
  • Thrombocytopenia, agranulocytosis, pancytopenia
  • Anaphylactic shock, malaise, fever
  • Bronchospasm, encephalopathy, hepatitis
  • Increases in liver enzymes

Pregnancy & Lactation

  • No adverse effects on pregnancy or fetus/newborn child
  • Use during pregnancy is acceptable
  • No information on passage into breast milk or effects on the neonate
  • Breast-feeding should be discontinued if essential

Precautions & Warnings

  • Avoid concomitant use with clopidogrel
  • Increased risk for osteoporosis-related fractures
  • Atrophic gastritis noted in long-term treatment
  • Concomitant use with methotrexate may lead to toxicities
