Indications
- Acute bacterial exacerbation of chronic bronchitis in adults
- Acute maxillary sinusitis
- Community-acquired pneumonia
- Pharyngitis or tonsillitis
- Uncomplicated skin and skin structure infections
- Acute otitis media in pediatric patients
- Treatment and prophylaxis of disseminated mycobacterial infections
- Helicobacter pylori infection and duodenal ulcer disease in adults
Pharmacology
- Inhibition of microsomal protein synthesis in susceptible organisms
- Binding to the donor site on the 50S subunit of the bacterial ribosome
- Activity against most Gram-positive bacteria and Chlamydia, some Gram-negative bacteria and Mycoplasmas
- Acid-stable and better tolerated compared to Erythromycin
- Twice the activity of Erythromycin against H. influenzae
- Most species of Gram-negative bacteria are resistant to Clarithromycin
Dosage & Administration
- Dosage varies based on condition and patient group
- Combination dosing regimens for H. pylori infection
- Recommended dosages for adults and children
- Adjustment in patients with renal and hepatic impairment
Interaction
- Contraindicated with specific medications such as HMG-CoA reductase inhibitors, gastroprokinetic agents, ergot alkaloids, antipsychotics, anti-gout agents, and antiarrhythmics
- Caution required with certain drug interactions
Contraindications
- Known hypersensitivity to clarithromycin, erythromycin, or any macrolide antibacterial drugs
- History of cholestatic jaundice or hepatic dysfunction associated with prior use of clarithromycin
Side Effects
- Nausea, vomiting, diarrhea, and abdominal pain
- Stomatitis and glossitis
- Headache, allergic reactions, taste perversion
- Transient central nervous system side effects including anxiety, dizziness, insomnia, and hallucination
Pregnancy & Lactation
- Use in neonates and children in appropriate doses
- Breast milk from mothers receiving Clarithromycin should not be given to infants until treatment is completed
Precautions & Warnings
- Discontinue if severe acute hypersensitivity reactions occur
- Avoid in patients with known QT prolongation or receiving drugs known to prolong the QT interval
- Discontinue if signs and symptoms of hepatitis occur
- Increased risk of all-cause mortality in patients with coronary artery disease post-treatment
- Evaluate for Clostridium difficile associated diarrhea if diarrhea occurs
Use in Special Populations
- Safety and effectiveness established for various conditions in pediatric patients
- Limited safety data for children under 6 months of age
Overdose Effects
- Ingestion of large amounts can produce gastro-intestinal symptoms
- Altered mental status, paranoid behavior, hypokalemia, and hypoxemia observed in extreme overdose case
- Prompt elimination of unabsorbed drug and supportive measures recommended for overdose reactions
