Indications

  • Treatment of gastric and duodenal ulcers
  • Treatment of NSAID-associated duodenal and gastric ulcer
  • Prophylaxis in patients with history of NSAID-associated duodenal and gastric ulcer
  • Treatment of gastro-esophageal reflux disease
  • Long-term management of acid reflux disease
  • Treatment of acid-related dyspepsia
  • Treatment of severe ulcerating reflux esophagitis
  • Prophylaxis of acid aspiration during general anesthesia
  • Treatment of Zollinger-Ellison syndrome
  • Treatment of Helicobacter pylori-induced peptic ulcer

Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of effect within one hour
  • Maximum effect within two hours
  • Inhibition of secretion lasts up to 72 hours

Dosage

  • 20 mg once daily (benign gastric and duodenal ulcer)
  • 20 mg once daily (NSAID-associated duodenal or gastric ulcer)
  • 20 mg once daily (gastro-esophageal reflux disease)
  • 10-20 mg daily (long-term management of acid reflux disease)
  • 10-20 mg once daily (acid-related dyspepsia)
  • 40 mg on the preceding evening, then 40 mg 2-6 hours before surgery (prophylaxis of acid aspiration)

Administration

  • Intravenous administration only
  • Must be given as a slow intravenous injection or infusion
  • Reconstituted solution should be used within specific timeframes

Interaction

  • Decreased intragastric acidity affecting ketoconazole absorption
  • Metabolized in the liver through cytochrome P450
  • Potential interactions with diazepam, phenytoin, warfarin, clarithromycin, and other drugs

Contraindications

  • Known hypersensitivity to omeprazole
  • Presence of suspected gastric ulcer malignancy

Side Effects

  • Skin rash, urticaria, pruritus
  • Photosensitivity, bullous eruption, erythema multiforme, angioedema, alopecia
  • Diarrhoea, headache, constipation, nausea/vomiting, flatulence, abdominal pain
  • Dry mouth, stomatitis, candidiasis
  • Paraesthesia, dizziness, light headedness, feeling faint
  • Somnolence, insomnia, vertigo
  • Reversible mental confusion, agitation, depression, hallucinations
  • Arthritic and myalgic symptoms
  • Blurred vision, taste disturbance, peripheral edema, increased sweating, gynaecomastia
  • Leucopenia, thrombocytopenia, agranulocytosis, pancytopenia
  • Anaphylactic shock, malaise, fever, bronchospasm
  • Hepatitis, interstitial nephritis, hepatic failure

Pregnancy & Lactation

  • No adverse effects of omeprazole on pregnancy or fetal health based on studies
  • Usage in pregnancy deemed safe
  • No information available on passage of omeprazole into breast milk
  • Breastfeeding should be discontinued if essential to use omeprazole

Precautions & Warnings

  • Avoid concomitant use with clopidogrel
  • Potential association with increased risk for osteoporosis-related fractures
  • Atrophic gastritis noted occasionally in long-term users
  • Potential for methotrexate toxicities with concomitant use
