Indications
- Treatment of anxiety states
- Treatment of anxiety associated with depression
- As adjunctive therapy in patients with schizophrenia
- Adjunctive therapy in patients with epilepsy
Pharmacology
- Binds at distinct binding sites associated with the chloride ionopore at the post-synaptic GABA receptor
- Increases duration of time for which the chloride ionopore is open
- Enhances post-synaptic inhibitory effect of GABA
Dosage
- Dosage and duration must be adjusted to the indication and severity of the condition
- Initial dose usually 20mg clobazam daily for adults and adolescents over 15 years
- Maintenance dose of 10 to 15mg clobazam daily for the elderly
- Daily dose of 5 to 10mg clobazam for children from 3 to 15 years
Administration
- Tablets can be administered whole or crushed and mixed in applesauce
- 10mg tablets can be divided into equal halves of 5mg
- Can be given with or without food
Interaction
- Concomitant consumption of alcohol can increase bioavailability by 50%
- Mutually potentiating effect with other central nervous system depressant drugs
- Increased risk of sedation, respiratory depression, and coma when used concomitantly with opioids
- Dosage adjustment when administered simultaneously with anticonvulsants
- Enhanced euphoria when used with narcotic analgesics
- Effects of muscle relaxants and nitrous oxide may be enhanced
Contraindications
- Hypersensitivity to clobazam or any excipients
- Myasthenia gravis
- Severe respiratory insufficiency
- Sleep apnoea syndrome
- Severe impairment of liver function
- Breastfeeding
Side Effects
- Decreased appetite
- Irritability, aggression, restlessness, depression
- Somnolence, sedation, dizziness, disturbance in attention
- Respiratory depression, respiratory failure
- Dry mouth, nausea, constipation
- Rash, photosensitivity reaction, urticaria
- Muscle spasms, muscle weakness
- Fatigue, slow response to stimuli, hypothermia
- Increased weight (particularly with high doses or in long-term treatment)
Pregnancy & Lactation
- Not recommended during the first trimester of pregnancy
- Clobazam should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus
- Not recommended in breastfeeding women
Precautions & Warnings
- Closely monitor for signs or symptoms of Stevens-Johnson syndrome
- Monitor respiratory function in patients with chronic or acute respiratory insufficiency
- Contraindicated in patients with myasthenia gravis
- Increased susceptibility to adverse effects in patients with renal or hepatic impairment
- Dose reduction recommended in elderly patients
- Consideration for the possibility of a decrease in anticonvulsant efficacy in the course of treatment
- Dosage adjustment in CYP2C19 poor metabolizers
- Increased incidence of suicide and suicide attempt in patients treated with benzodiazepines and hypnotics
- Increased exposure to N-desmethylNoanxit when used with CYP2C19 inhibitors
