Indications
- Psoriasis
- Recalcitrant dermatoses
- Lichen planus
- Discoid lupus erythematosus
- Other skin conditions which do not respond satisfactorily to less potent steroids
Composition
- Clobetasol Propionate Cream: Clobetasol Propionate BP 0.5 mg (0.05% w/w)
- Clobetasol Propionate Ointment: Clobetasol Propionate BP 0.5 mg (0.05% w/w)
- Clobetasol Propionate Scalp Application: Clobetasol Propionate BP 0.5 mg (0.05% w/w)
- Clobetasol Propionate Shampoo: Clobetasol Propionate BP 0.5 mg (0.05% w/w)
- Clobetasol Propionate Lotion: Clobetasol Propionate BP 0.5 mg (0.05% w/w)
- Clobetasol Propionate Spray: Clobetasol Propionate BP 0.5 mg (0.05% w/w)
Pharmacology
- Anti-inflammatory
- Antipruritic
- Vasoconstrictive
- Inhibits late phase allergic reactions
- Decreases mast cell density
- Inhibits cytokine production
Dosage
- Cream, Ointment, and Spray: Apply a thin layer twice daily
- Occlude for enhanced effect in resistant lesions
- Super-high potency topical corticosteroids
- Limited to 2 consecutive weeks
- Maximum weekly dose should not exceed 50 gm
- Scalp Solution and Shampoo: Apply once daily to affected areas
- Total dosage should not exceed 50 g per week
- Discontinue if no response after one week
Administration
Cutaneous. Creams for moist or weeping surfaces, ointments for dry or scaly lesions
Interaction
Co-administered drugs that can inhibit CYP3A4 can increase systemic exposure
Contraindications
- Rosacea
- Acne vulgaris
- Perioral dermatitis
- Primary cutaneous viral infections
- Hypersensitivity to the preparation
- Not indicated for primarily infected skin lesions or dermatoses in children under one year of age
Side Effects
- Hypercorticism with prolonged use
- Local atrophic changes in the skin
- Provocation of pustular form of psoriasis
- Pigmentation changes
- Hypertrichosis
- Hypersensitivity
Pregnancy & Lactation
Limited data on use in pregnant women, caution in lactating women
Precautions & Warnings
- Avoid long-term continuous therapy, especially in infants and children
- Risk of atrophic changes on the face
- Care needed to prevent entry into the eyes
- Hazardous in psoriasis
- Use of anti-microbial therapy for infected lesions
Use in Special Populations
Avoid long-term continuous therapy in infants and children under 12 years of age
Overdose Effects
Acute overdosage is unlikely, features of hypercortisolism in chronic over-dosage
