Indications
- Treatment of gastric and duodenal ulcer
- NSAID-associated duodenal and gastric ulcer treatment
- Prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcer
- Treatment of gastro-esophageal reflux disease
- Long-term management of acid reflux disease
- Treatment of acid-related dyspepsia
- Treatment of severe ulcerating reflux esophagitis
- Prophylaxis of acid aspiration during general anesthesia
- Treatment of Zollinger-Ellison syndrome
- Helicobacter pylori-induced peptic ulcer treatment
Pharmacology
- Inhibitor of gastric acid secretion
- Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
- Onset of antisecretory effect within one hour
- Maximum effect occurring within two hours
- Inhibition of secretion lasts up to 72 hours
- Gradual return of secretory activity over 3 to 5 days after discontinuation
Dosage
- 20 mg once daily for benign gastric and duodenal ulcer for 4-8 weeks
- Maintenance dose for recurrent duodenal ulcer 20 mg once daily
- 10-20 mg daily for long-term management of acid reflux disease
- 40 mg once daily for prophylaxis of acid aspiration during general anesthesia
- 60 mg once daily for Zollinger-Ellison syndrome
- 20 mg twice daily for Helicobacter pylori eradication regimen
Administration
- Omeprazole IV injection should be given as a slow intravenous injection
- Omeprazole IV infusion should be given over a period of 20-30 minutes or more
- Reconstituted solutions should be used within specified timeframes
Interaction
- Decreased intragastric acidity affecting absorption of ketoconazole
- Metabolism through cytochrome P450 impacting elimination of diazepam, phenytoin, and warfarin
- Possible interaction with Proceptin and clarithromycin
- No evidence of interaction with various medications
Contraindications
- Known hypersensitivity to Omeprazole
- Suspected gastric ulcer unless malignancy is excluded
Side Effects
- Well-tolerated with generally mild and reversible adverse reactions
- Reports of skin rash, urticaria, pruritus, photosensitivity, and alopecia
- Gastrointestinal reactions including diarrhea, headache, constipation, nausea/vomiting
- Other reported reactions like dry mouth, dizziness, paraesthesia, and mental confusion
- Reports of arthritic and myalgic symptoms, vision disturbance, and increased sweating
- Observations of liver enzymes increases
Pregnancy & Lactation
- No adverse effects indicated on pregnancy or health of the fetus/newborn child
- No information available on the passage of Omeprazole into breast milk or its effects on the neonate
- Breastfeeding discontinuation recommended if essential use of Omeprazole
Precautions & Warnings
- Avoid concomitant use of clopidogrel and Proceptin
- Increased risk for osteoporosis-related fractures with PPI therapy
- Atrophic gastritis noted occasionally in long-term treatment
- Concomitant use with methotrexate may lead to methotrexate toxicities
