Indications
- Reduces rate of Myocardial Infarction (MI) and Stroke in patients with non-ST-segment elevation ACS
- Reduces rate of MI and Stroke in patients with established peripheral arterial disease
- Reduces rate of MI and Stroke in patients with history of recent MI or recent Stroke
Pharmacology and Pharmacokinetics
- Clopidogrel is a prodrug
- Inhibits platelet activation and aggregation
- Irreversibly binds to the P2Y12 class of ADP receptors on platelets
- Dose-dependent inhibition of platelet aggregation
- Rapid absorption after oral administration
- Plasma concentrations of parent compound are very low and generally below quantification limit beyond 2 hours after dosing
- Extensively metabolized by the liver
- Main circulating metabolite has no effect on platelet aggregation
- About 50% excreted in urine, 46% in faeces
- Elimination half-life of main metabolite is 8 hours
- Clopidogrel binds reversibly to human plasma proteins
- Undergoes rapid hydrolysis into carboxylic acid derivative
Dosage & Administration
- Recommended dose is 75 mg once daily with or without food
- Initiate with a single 300 mg loading dose for acute coronary syndrome
- Aspirin should be initiated and continued in combination with Clopidogrel
Interaction
- Aspirin does not modify the Clopidogrel-mediated inhibition of ADP-induced platelet aggregation
- Clopidogrel potentiates the effect of aspirin on collagen-induced platelet aggregation
- Co-administration with heparin has no effect on platelet aggregation induced by Clopidogrel
- Increased occult gastrointestinal blood loss with NSAIDs
- Caution advised with concomitant warfarin administration
Contraindications
- Hypersensitivity to the drug substance or any component of the product
- Active pathological bleeding such as peptic ulcer or intracranial hemorrhage
Side Effects
- Generally well tolerated
Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- Safe during first and second trimesters only if clearly needed
- Contraindicated during the third trimester
- Unknown excretion of Clopidogrel in human breast milk
- Aspirin known to be excreted in human milk
- Discontinue during breastfeeding
Precautions & Warnings
- May prolong bleeding time
- Rare reports of Thrombotic thrombocytopenic purpura (TTP)
- Potential development of Reye's syndrome in individuals with certain conditions
- Hypersensitivity reactions reported
Use in Special Populations
- Not recommended for children under 12 years unless expected benefits outweigh possible risks
- Aspirin may be a contributory factor in the causation of Reye's syndrome in some children
Overdose Effects
- May lead to bleeding complications
- Platelet transfusion may restore clotting ability
- Aspirin intoxication may lead to various symptoms which can be treated through induced vomiting, gastric lavage, and other symptomatic treatments
