Description
- Clopidogrel and Aspirin combination for the treatment of Acute Coronary Syndrome (ACS)
- Anti-platelet medication for reducing the rate of Myocardial Infarction (MI) and Stroke
Dosage
- Recommended dose of 75 mg once daily with or without food
- Initial loading dose of 300 mg for patients with acute coronary syndrome
Mode of Action
- Clopidogrel inhibits platelet activation and aggregation through irreversible binding to P2Y12 receptors
- Aspirin inhibits platelet aggregation by irreversibly inhibiting cyclooxygenase
Pharmacokinetics
- Rapid absorption of Clopidogrel after oral administration
- Metabolized by the liver with a half-life of 8 hours
Interaction
- No modification of Clopidogrel-mediated inhibition by Aspirin
- Increased occult gastrointestinal bleeding with NSAIDs
Contraindications
- Hypersensitivity to the drug or any component
- Active pathological bleeding conditions
Side Effects
- Generally well tolerated
Pregnancy & Lactation
- Use during first and second trimesters if necessary
- Discontinue during the third trimester and breastfeeding
Warnings
- May prolong bleeding time
- Rare reports of Thrombotic thrombocytopenic purpura (TTP)
Special Populations
- Not recommended for children under 12 years
- Aspirin caution in Reye's syndrome development
Overdose Effects
- Bleeding complications with Clopidogrel overdose
- Treatment for aspirin intoxication includes inducing vomiting and gastric lavage if needed
