Indications
- Reduces rate of Myocardial Infarction (MI) and Stroke in non-ST-segment elevation ACS
- Reduces rate of MI and Stroke in recent MI, recent Stroke, or established Peripheral Arterial Disease
- Reduces rate of MI and Stroke in patients with history of recent MI or recent Stroke
Pharmacology
- Clopidogrel is a prodrug
- Inhibits platelet activation and aggregation
- Irreversibly binds to P2Y12 class of ADP receptors
- Dose-dependent inhibition of platelet aggregation
- Aspirin inhibits platelet aggregation by irreversible inhibition of platelet cyclooxygenase
- Inhibits the generation of thromboxane A2
Dosage & Administration
- Recommended dose is 75 mg once daily
- Initiate with a single 300 mg loading dose for acute coronary syndrome
- Should be continued with 75 mg once daily
- Aspirin should be initiated and continued in combination
Interaction
- Aspirin potentiates the effect of Clopidogrel on collagen-induced platelet aggregation
- Heparin has no effect on inhibition of platelet aggregation induced by Clopidogrel
- NSAIDs and Clopidogrel should be co-administered with caution
- Concomitant administration of Clopidogrel with warfarin should be undertaken with caution
Contraindications
- Hypersensitivity to the drug substance or any component
- Active pathological bleeding such as peptic ulcer or intracranial hemorrhage
Side Effects
- Generally well tolerated
Pregnancy & Lactation
- Use during first and second trimesters if clearly needed
- Contraindicated during the third trimester
- Unknown excretion of Clopidogrel in human breast milk
Precautions & Warnings
- May prolong bleeding time
- Thrombotic thrombocytopenic purpura (TTP) has been reported rarely
- Reye's syndrome may develop
- Hypersensitivity including rash, angioedema or hematologic reaction
Use in Special Populations
- Not recommended for children, particularly those under 12 years
Overdose Effects
- Clopidogrel overdose may lead to bleeding complications
- Platelet transfusion may restore clotting ability
- Aspirin overdose may cause dizziness, headache, tinnitus, confusion
- Can be treated by inducing vomiting followed by gastric lavage if needed
