Indications

  • Reduces rate of Myocardial Infarction (MI)
  • Reduces rate of Stroke
  • Treatment for non-ST-segment elevation ACS

  • Reduces rate of MI in patients with recent MI or established Peripheral Arterial Disease
  • Reduces rate of Stroke in patients with recent Stroke or established Peripheral Arterial Disease

Pharmacology

  • Inhibits platelet activation
  • Inhibits platelet aggregation
  • Binds to P2Y12 class of ADP receptors
  • Dose-dependent inhibition of platelet aggregation
  • Rapid absorption after oral administration
  • Inhibition of platelet aggregation by irreversible inhibition of platelet cyclooxygenase

Dosage & Administration

  • Recommended dose of 75 mg once daily
  • Initiation with single 300 mg loading dose (acute coronary syndrome)
  • Combination therapy with Aspirin
  • No dosage adjustment for elderly patients
  • No dosage adjustment for patients with renal impairment

Interaction

  • Does not modify Clopidogrel-mediated inhibition of ADP-induced platelet aggregation (Aspirin)
  • Potentiates effect of aspirin on collagen-induced platelet aggregation
  • Does not alter the effect of heparin on coagulation

Contraindications

  • Hypersensitivity to the drug substance
  • Active pathological bleeding such as peptic ulcer or intracranial hemorrhage

Side Effects

  • Generally well tolerated

Pregnancy & Lactation

  • Should be used during first and second trimesters if clearly needed
  • Contraindicated during the third trimester of pregnancy
  • Unknown excretion in human breast milk (Clopidogrel)
  • Excreted in human milk (Aspirin)
  • Discontinue during breastfeeding

Precautions & Warnings

  • May prolong bleeding time
  • Reported rare cases of Thrombotic thrombocytopenic purpura (TTP)
  • Reye's syndrome caution in individuals with chicken pox, influenza or flu symptoms
  • Risk of hypersensitivity reactions

Use in Special Populations

  • Not recommended for children under 12 years
  • Risks associated with age-related conditions

Overdose Effects

  • May lead to bleeding complications
  • Moderate Aspirin intoxication effects
  • Severe Aspirin intoxication effects
  • Treatment with platelet transfusion
