Indications

Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of antisecretory effect within one hour
  • Maximum effect within two hours
  • Inhibition of secretion lasts up to 72 hours
  • Gradual return of secretory activity over 3 to 5 days after discontinuation

Dosage

  • Benign gastric and duodenal ulcer: 20mg once daily for 4-8 weeks
  • NSAID-associated duodenal or gastric ulcer: 20mg once daily for 4-8 weeks
  • Gastro-esophageal reflux disease: 20mg once daily for 4-8 weeks
  • Long-term management of acid reflux disease: 10-20mg daily
  • Acid-related dyspepsia: 10-20mg once daily for 2-4 weeks
  • Zollinger-Ellison syndrome: 20-120mg daily; 2 divided doses if daily dose is more than 80mg
  • Helicobacter pylori eradication regimen: 20mg twice daily with antimicrobial agents
  • Paediatric use in severe ulcerating reflux esophagitis: 10-40mg once daily for 4-12 weeks

Administration

  • IV Injection for prophylaxis of acid aspiration: 40mg given slowly over 5 minutes
  • IV Injection for duodenal ulcer, gastric ulcer or reflux esophagitis: 40mg once daily
  • IV Injection for Zollinger-Ellison syndrome: 60mg daily; higher doses may be required
  • IV Infusion over 20-30 minutes or more

Interaction

  • Reduced absorption of ketoconazole during treatment
  • Metabolised in the liver through cytochrome P450
  • Potential delay in elimination of diazepam, phenytoin, and warfarin
  • Increased plasma concentrations during concomitant administration with clarithromycin
  • No evidence of interaction with phenacetin, theophylline, caffeine, propranolol, and more

Contraindications

  • Known hypersensitivity to omeprazole
  • Suspicion of gastric ulcer without excluding possibility of malignancy

Side Effects

  • Skin rash, urticaria, pruritus, photosensitivity
  • Diarrhea, headache, constipation, nausea/vomiting
  • Dry mouth, stomatitis, candidiasis, paraesthesia
  • Dizziness, lightheadedness, somnolence, insomnia
  • Reversible mental confusion, agitation, depression, hallucinations
  • Arthritic and myalgic symptoms, blurred vision, taste disturbance
  • Peripheral edema, increased sweating, leucopenia, thrombocytopenia
  • Hepatitis, interstitial nephritis, hepatic failure, increases in liver enzymes

Pregnancy & Lactation

  • No adverse effects on pregnancy or fetus/newborn child
  • Safe for use during pregnancy
  • No information on passage into breast milk or effects on neonate
  • Discontinue breast-feeding if essential use of omeprazole

Precautions & Warnings

  • Avoid concomitant use with clopidogrel
  • Potential association with increased risk for osteoporosis-related fractures
  • Atrophic gastritis noted in long-term treatment
  • Concomitant use with methotrexate may lead to toxicities
