Indications
- Reduces the rate of Myocardial Infarction (MI)
- Reduces the rate of Stroke
- For patients with non-ST-segment elevation ACS
- For patients with acute ST-segment elevation ACS
- For patients with recent MI
- For patients with recent Stroke
- For patients with established Peripheral Arterial Disease
Pharmacology
- Inhibits platelet activation
- Inhibits platelet aggregation
- Dose-dependent inhibition of platelet aggregation
- Irreversible binding to P2Y12 class of ADP receptors
- Metabolized in the liver
- Rapid hydrolysis into carboxylic acid derivative
Dosage & Administration
- Recommended dose: 75 mg once daily
- Initiate with a single 300 mg loading dose for acute coronary syndrome
- May be taken with or without food
- No dosage adjustment for elderly patients
Interaction
- Clopidogrel potentiates the effect of aspirin on collagen-induced platelet aggregation
- No effect of heparin on inhibition of platelet aggregation induced by Clopidogrel
- Increased occult gastrointestinal blood loss when co-administered with NSAIDs
Contraindications
- Hypersensitivity to the drug substance or any component
- Active pathological bleeding such as peptic ulcer or intracranial hemorrhage
Side Effects
- Generally well tolerated
Pregnancy & Lactation
- Should be used during first and second trimesters only if clearly needed
- Contraindicated during the third trimester
- Unknown excretion in human breast milk
Precautions & Warnings
- May prolong bleeding time
- Rare risk of Thrombotic thrombocytopenic purpura (TTP)
- Reye's syndrome may develop in individuals with certain symptoms
- Hypersensitivity reactions reported
Use in Special Populations
- Should not be given to children under 12 years unless benefits outweigh risks
- Aspirin may be a contributory factor in Reye’s syndrome in some children
Overdose Effects
- May lead to bleeding complications
- Dizziness, headache, tinnitus, confusion, and gastrointestinal symptoms in moderate aspirin intoxication
- Respiratory alkalosis, respiratory acidosis, metabolic acidosis, hyperthermia, dehydration in severe aspirin intoxication
