- Reduce the rate of MI in patients with established peripheral arterial disease
- Reduce the rate of Stroke in patients with established peripheral arterial disease
- Reduce the rate of MI in patients with a history of recent MI
- Reduce the rate of Stroke in patients with a history of recent Stroke
Pharmacology
- Active metabolite binds to P2Y12 class of ADP receptors
- Dose-dependent inhibition of platelet aggregation
- Inhibition of platelet cyclooxygenase by Aspirin
- Inhibition of generation of thromboxane A2
- Rapid absorption after oral administration
- Plasma concentrations of the parent compound are very low
- Extensive liver metabolism
- Approximately 50% excreted in urine
- Approximately 46% excreted in faeces
- Elimination half-life of the main circulating metabolite is 8 hours
- Linear pharmacokinetics
Dosage & Administration
- Recommended dose: 75 mg once daily
- Can be taken with or without food
- Recommended loading dose for acute coronary syndrome: single 300 mg dose
- Combination with Aspirin (75 mg-325 mg once daily)
Interaction
- No modification of Clopidogrel-mediated inhibition by Aspirin
- Potentiation of Aspirin effect on collagen-induced platelet aggregation by Clopidogrel
- No modification of heparin dose by Clopidogrel
- No alteration of heparin effect on coagulation by Clopidogrel
- Increased occult gastrointestinal blood loss with NSAIDs
- Caution with co-administration of NSAIDs and Clopidogrel
- Safety of co-administration with warfarin not established
Contraindications
- Hypersensitivity to the drug substance or any component
- Active pathological bleeding such as peptic ulcer or intracranial hemorrhage
Side Effects
- Generally well tolerated
Pregnancy & Lactation
- Limited studies in pregnant women
- Use during first and second trimesters if clearly needed
- Contraindicated during third trimester
- Unknown excretion in human breast milk
Precautions & Warnings
- Prolongs bleeding time
- Risk of Thrombotic thrombocytopenic purpura (TTP)
- Risk of Reye's syndrome in children with chicken pox or influenza
- History of hypersensitivity to thienopyridines
Use in Special Populations
- Not recommended for children under 12 years
- Risk of Reye’s syndrome in children
Overdose Effects
- May lead to bleeding complications
- Symptoms of moderate aspirin intoxication
- Symptoms of severe aspirin intoxication
- Treatment with platelet transfusion for Clopidogrel overdose
- Treatment with induced vomiting and gastric lavage for aspirin intoxication
- Treatment with haemodialysis and symptomatic measures for severe aspirin intoxication
