Indications
- Acute Coronary Syndrome (ACS)
- Recent MI
- Recent Stroke
- Established Peripheral Arterial Disease
Pharmacology
Inhibits platelet activation and aggregation through irreversible binding of its active metabolite to P2Y12 class of ADP receptors on platelets
Dosage & Administration
- 300 mg oral loading dose for acute coronary syndrome
- 75 mg once daily for recent MI, recent stroke, or established peripheral arterial disease
- Given orally with or without food
Interaction
- Increases risk of bleeding with NSAIDs, warfarin, SSRIs, SNRIs
- Avoid concomitant use with CYP2C19 inhibitors (omeprazole or esomeprazole)
- Increases plasma concentrations of Repaglinide when used with Loplate
Contraindications
- Hypersensitivity to the drug
- Active pathological bleeding
Side Effects
- Common side effects: Bleeding, Diarrhoea, Gastrointestinal discomfort, Haemorrhage, Skin reactions
- Rare side effects: Acquired haemophilia, Anaemia, Angioedema, Arthralgia, Arthritis, Bone marrow disorders
Pregnancy & Lactation
Use during pregnancy only if clearly needed, unknown excretion in human breast milk
Precautions & Warnings
- Metabolism impairment with genetic variations in CYP2C19
- Increased risk of bleeding
- Discontinuation increases risk of cardiovascular events
- Reported hypersensitivity reactions
Use in Special Populations
Safety and effectiveness not established in pediatric populations, no dosage adjustment necessary in elderly patients
Overdose Effects
May lead to bleeding complications, platelet transfusion may restore clotting ability
