Indications
- Gastric and duodenal ulcer
- NSAID-associated duodenal and gastric ulcer
- Prophylaxis in patients with a history of NSAID-associated ulcers
- Gastro-esophageal reflux disease
- Long-term management of acid reflux disease
- Acid-related dyspepsia
- Severe ulcerating reflux esophagitis
- Prophylaxis of acid aspiration during general anesthesia
- Zollinger-Ellison syndrome
- Helicobacter pylori-induced peptic ulcer
Pharmacology
- Inhibitor of gastric acid secretion
- Gastric parietal cell
- Hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
- Onset of antisecretory effect within one hour
- Maximum effect within two hours
- Inhibition of secretion lasts up to 72 hours
- Gradual return of secretory activity over 3 to 5 days after discontinuation
Dosage
- 20 mg once daily for 4-8 weeks in benign gastric and duodenal ulcer treatment
- 20 mg once daily for 4 weeks in NSAID-associated duodenal or gastric ulcer treatment
- 10-20 mg daily for long-term management of acid reflux disease
- 40 mg on the preceding evening and 2-6 hours before surgery for prophylaxis of acid aspiration
- Initially 60 mg once daily for Zollinger-Ellison syndrome
- 20 mg twice daily in helicobacter pylori eradication regimen in peptic ulcer disease
- 10-20 mg once daily for 4-12 weeks for severe ulcerating reflux esophagitis in pediatric use
Administration
- Intravenous administration only for Omeprazole lyophilized powder and water for injection
- IV injection given slowly over a period of 5 minutes
- IV infusion given over a period of 20-30 minutes
- Reconstituted solution should be used within specific time frames and not mixed with other drugs in the same infusion set
Interaction
- Decreased intragastric acidity affecting ketoconazole absorption
- Metabolism through cytochrome P450 affecting elimination of diazepam, phenytoin, and warfarin
- Plasma concentrations of Omeprazole and clarithromycin increased during concomitant administration
- No evidence of interaction with phenacetin, theophylline, caffeine, or antacids
- Increased bioavailability of digoxin with simultaneous treatment with Omeprazole
Contraindications
- Known hypersensitivity to Omeprazole
- Exclusion of malignancy possibility in suspected gastric ulcer before treatment initiation
Side Effects
- Skin rash, urticaria, and pruritus
- Photosensitivity, bullous eruption, and erythema multiforme
- Diarrhea, headache, constipation, and nausea/vomiting
- Dry mouth, stomatitis, and candidiasis
- Paraesthesia, dizziness, and light headedness
- Reversible mental confusion, agitation, and depression
- Arthritic and myalgic symptoms
- Increases in liver enzymes
Pregnancy & Lactation
- No adverse effects of Omeprazole on pregnancy or fetus/newborn child found in prospective epidemiological studies
- Use during pregnancy can be considered
- No information available on passage into breast milk or effects on neonate
- Discontinuation of breastfeeding advised if essential use of Omeprazole is considered
Precautions & Warnings
- Avoid concomitant use of clopidogrel and Omeprazole
- Possible increased risk for osteoporosis-related fractures with PPI therapy
- Atrophic gastritis noted occasionally with long-term use
- Concomitant use with methotrexate may lead to toxicities
