Description
- Glophen D is an ophthalmic solution indicated for various eye conditions such as acute purulent conjunctivitis, corneal ulcers, keratitis, otitis externa, and otitis media. Pharmacology
- Dexamethasone has anti-inflammatory and anti-allergic action on the anterior segment of the eye
- Chloramphenicol is absorbed rapidly after administration and exerts its antibacterial effect by inhibiting bacterial protein synthesis
Dosage & Administration
- For bacterial conjunctivitis: 1-2 drops every 2 hours for 2 days then 1-2 drops every 4 hours for the next 5 days
- For corneal ulcers: 2 drops every 15 minutes for 6 hours, then 2 drops every 30 minutes for the rest of the first day, followed by hourly instillation on the second day, and subsequent 4-hour intervals for 3-14 days
- For otitis externa and otitis media: 2-3 drops every 2-3 hours initially, with decreased frequency as clinical signs improve
Interaction
- Increase in anticoagulant effect when absorbed with warfarin
- Inhibition of Chloramphenicol's action when given with Phenobarbital
- Reduced efficacy of Dexamethasone with phenytoin, phenobarbitone, ephedrine, and rifampicin
Contraindications
- Epithelial herpes simplex cornealis
- Fungal, viral, or tuberculous infections of the eye
- Glaucoma
- Myelosuppression during previous Chloramphenicol exposure
- Hypersensitivity to Chloramphenicol or Dexamethasone Phosphate
Side Effects
- Transient ocular burning or discomfort
- Stinging, redness, itching, conjunctivitis
- Foreign body sensation, photophobia, blurred vision
- Dryness and eye pain
- Elevation of intraocular pressure with possible glaucoma
Pregnancy & Lactation
- Caution in pregnancy and nursing mothers, with use only if potential benefit justifies potential risk
Precautions & Warnings
- Consideration of persistent fungal infections with prolonged corticosteroid dosing
- Potential for posterior subcapsular cataract formation and glaucoma with prolonged steroid use
- Monitoring of intraocular pressure is needed
- Possible overgrowth of non-susceptible organisms with prolonged antibiotic use
- Discontinue if new infections appear and institute alternative therapy
