Pharmacology
- Attaches to specific site on the GABA receptor
- Potentiates the effect of GABA
- Opens chloride ion channels into cells
- Rapid absorption after oral administration
- Peak plasma concentration reached within 15-90 minutes
- Mean plasma half-life of 30 hours
- 98-99% plasma protein binding
- Metabolized in the liver
- Traces of unchanged drug excreted in urine
- Metabolites excreted through bile into the intestine
- Eliminated with the feces
- Significant absorption after rectal administration
Dosage & Administration
- Oral administration for anxiety and insomnia
- IM/slow IV injection for severe acute anxiety and febrile convulsion
- Rectal administration for children
- Different doses for adults and children
- Increased dosage if necessary
Interaction
- Not recommended with alcohol
- Increased sedation with concomitant use of neuroleptics, hypnotics, sedative antihistamines and CNS depressants
- Enhanced clearance with phonobarbitone
- Decreased clearance with cimetidine
- Inhibition of metabolism by omeprazole and isoniazide
Contraindications
- Myasthenia gravis
- Pulmonary insufficiency
- Respiratory depression
- Hypersensitivity to benzodiazepine

Pregnancy & Lactation
- Crosses placental barrier

Precautions & Warnings
- Avoid prolonged use and abrupt withdrawal
- Caution in respiratory disease and muscle weakness
- Use with caution in history of drug or alcohol abuse
- Use with caution in cases of hepatic or renal impairment
Overdose Effects
- Sedation
- Muscle weakness
- Profound sleep or paradoxical excitation
- More severe effects may include ataxia, hypotonia, hypotension, and respiratory depression
- Rarely coma and death
