Indications

Pharmacology

  • Potent nonsteroidal anti-inflammatory
  • Analgesic properties
  • Antipyretic properties
  • Inhibition of prostaglandin synthesis
  • Peak plasma concentration achieved within half an hour

Dosage & Administration

  • Adult dosage: once daily intramuscular injection
  • Severe cases may require twice daily dosage
  • Renal colic dosage: once daily intramuscularly, second dose after 30 minutes if necessary
  • Children dosage: 1-3 mg per kg body weight daily in divided doses
  • Recommended lowest effective dosage for elderly patients

Interaction

  • Increased plasma concentrations of Lithium and Digoxin
  • Risk of haemorrhage with anticoagulant therapy
  • No influence on antidiabetic agents' clinical effect
  • Cases of nephrotoxicity with concomitant use of Cyclosporin
  • Serious toxicity when used with Methotrexate
  • Possibility of convulsions with quinolone antimicrobials
  • Possible increase in unwanted effects with other NSAIDs and steroids
  • Potential to inhibit the activity of diuretics

Contraindications

  • Hypersensitivity to Diclofenac
  • Active or suspected peptic ulcer
  • Gastrointestinal bleeding
  • Hypersensitivity to local anaesthetics of amide type
  • Adams-Stokes syndrome or severe degrees of heart block
  • Hypersensitivity to any excipients used in the formulation

Side Effects

  • Mild and transient gastrointestinal discomfort
  • Nausea
  • Diarrhea
  • Bleeding
  • Injection site disorders
  • Abscesses
  • Local necrosis
  • CNS-related reactions such as drowsiness and dizziness

Pregnancy & Lactation

  • Not recommended unless compelling reasons exist
  • Lowest effective dosage to be used
  • Not recommended during the last trimester
  • Small quantities detected in breast milk with no expected undesirable effects on the infant

Precautions & Warnings

  • Close observation for patients with severe hepatic, cardiac, or renal insufficiency
  • Monitoring of renal function
  • Use of lowest effective dose
  • Monitoring of hepatic function tests
  • Monitoring of renal, hepatic function, and blood counts for long-term treatment
